The Patient Health Questionnaire (PHQ-9) is a nine-item, validated, self-reported measure of depression that is based
on the nine diagnostic criteria for depressive disorders according to the DSM-IV and yields a measure of depression
severity. Scores range from 0 to 27 and scores of 5, 10, 15 and 20 reflect mild, moderate, moderately severe and severe
depression, respectively.

depression in all neurological diseases, with the most severe depression in those with chronic pain.
