NORWAY: Most pharmaceutical products<br />

are reimbursed according to a system<br />

based on diagnoses and approved<br />

products prescribed by a physician. A<br />

condition is long-term need <strong>for</strong> the<br />

product, equipment or accessories.<br />

SWEDEN: A new rebate system <strong>for</strong> prescribed<br />

pharmaceutical products was introduced<br />

in <strong>1997</strong>. The rebate is calculated<br />

according to the value of the products<br />

bought. For purchases of up to SEK<br />

400 over a 12 month period, the user<br />

pays all. A rebate is given <strong>for</strong> exceeding<br />

costs. For costs between SEK 400 and<br />

1,200, there is a 50 per cent rebate. Between<br />

SEK 1,200 and 2,800 the rebate is<br />

75 per cent, and between SEK 2,800 and<br />

3,800 the rebate is 90 per cent. When<br />

products have been purchased <strong>for</strong> a sum<br />

of SEK 3,800, the ceiling of user charge<br />

has been reached. At this level, the patient<br />

will have paid SEK 1,300 and receives<br />

a free pass <strong>for</strong> the rest of the year.<br />

The scheme covers rebate approved<br />

medicines on prescription including contraceptives<br />

and accessories <strong>for</strong> stoma. Insulin<br />

is free of charge.<br />

ORGANIZATION OF HEALTH SERVICES<br />

NORGE: De fleste lægemidler refun<strong>de</strong>res<br />

efter et system baseret på diagnoser<br />

og godkendte præparater <strong>for</strong>skrevet af en<br />

læge. Udgangspunktet er at man langvarigt<br />

har behov <strong>for</strong> lægemidlet, medicinsk<br />

udstyr eller <strong>for</strong>brugsvarer.<br />

SVERIGE: I <strong>1997</strong> blev <strong>de</strong>r indført et<br />

nyt rabatsystem <strong>for</strong> lægemidler på recept.<br />

Rabatten udregnes efter værdien på <strong>de</strong><br />

lægemidler som købes. For indkøb op til<br />

400 SEK i en 12 måne<strong>de</strong>rs perio<strong>de</strong> betaler<br />

man selv <strong>de</strong>t hele. På udgifter <strong>de</strong>rudover<br />

y<strong>de</strong>s <strong>de</strong>r rabat. For udgifter mellem<br />

400 SEK og 1.200 SEK gives <strong>de</strong>r 50 pct.<br />

rabat. Mellem 1.200 og 2.800 SEK er<br />

rabatten 75 pct. og <strong>for</strong> udgifter mellem<br />

2.800 SEK og 3.800 SEK er rabatten 90<br />

pct. Når <strong>de</strong>r er købt lægemidler <strong>for</strong> 3.800<br />

SEK har man nået op på egenbetalingens<br />

maksimum. Patienten har ved <strong>de</strong>tte niveau<br />

selv betalt 1.300 SEK og får så til<strong>de</strong>lt<br />

et frikort <strong>for</strong> resten af året. Ordningen<br />

omfatter rabatberettige<strong>de</strong> lægemidler<br />

på recept, inkl. p-piller og brugsartikler<br />

til stomier. Insulin er gratis.<br />

Treatment at hospitals Behandlinger ved sygehuse<br />

As appears from the survey, there are no<br />

user charges <strong>for</strong> hospitalization in Denmark,<br />

the Faroe Islands, Greenland, Iceland<br />

and Norway. In Iceland, however,<br />

there is a charge <strong>for</strong> specialist polyclinic<br />

treatment at hospitals, cf the section on<br />

consultations with physician.<br />

FINLAND AND ÅLAND: Patients pay<br />

a charge <strong>for</strong> admission to hospital, psychiatric<br />

ward or health centre, which as<br />

per January 1 1999 was a maximum of<br />

FIM 125 per day. The payment <strong>for</strong> one<br />
