Use of Village health register for PIP
preparation. PIP need to be approved
by Gram Sabha. After approval,
forward the copy of PIP to MO PHC.
PIP to be prepared based on SC
format. PIP approved by SC Monitoring
Committee. PIP to be submitted to MO
PHC on 31.12.2010
Submission of Plans to EC and GB of
RKS for sanctioning. Once the plans are
approved by EC and GB, forward
approved plans to THO for preparation
of block plans.
All the block plans are prepared at
district HQ in district level workshop.
DPM will be responsible for
arrangement of workshop. Workshop
will be attended by MS, THO, MO.
There will be two sections of block
plan. Section ‐ I of the block plan will
consist of plan of all PHCs put together
+ THO HQ plan.
Section ‐ II of the plan will consist of
plan of all the RH/SDH in block put
together.
Block plan (both sections) to be
approved by Block Health Mission.
District Plan will be prepared by
addition of block plans, plans of
District and regional level hospitals
situated in district and District HQ
plan.
Workshop will be organised at
HFWTCs for preparation of District
PIPs during 10‐13 January 2011.
