Officer’s Certification and<br />

Contact Information<br />

An officer of your institution must review<br />

the completed LAR and certify it is<br />

accurate before submitting it <strong>to</strong> your<br />

processing agency. Certification is<br />

made on the transmittal sheet that will<br />

accompany the loan/application register.<br />

A checklist <strong>to</strong> assist the officer in the<br />

review preceding certification appears<br />

in this <strong>Guide</strong> on page 24. A checklist for<br />

the person completing the LAR appears<br />

on the next page.<br />

Verify that the transmittal sheet also<br />

includes the name, e-mail address,<br />

and telephone and facsimile numbers<br />

of a person at your institution who can<br />

answer questions about the report. Also<br />

verify that the transmittal sheet gives a<br />

record count of the <strong>to</strong>tal number of line<br />

entries on that particular submission. (If<br />

your institution has <strong>to</strong> resubmit data for<br />

any reason, the record count on the<br />

transmittal sheet must correspond <strong>to</strong><br />

that submission.) A completed transmittal<br />

sheet must be part of your <strong>HMDA</strong><br />

package whether you are required <strong>to</strong><br />

submit the loan data in au<strong>to</strong>mated format<br />

or are permitted <strong>to</strong> submit it in<br />

paper form.<br />

Remember that if your institution keeps<br />

separate registers for different<br />

branches or for different types of loans,<br />

all registers must be consolidated and<br />

submitted <strong>to</strong> your processing agency in<br />

a single package with one transmittal<br />

sheet.
