If there is an open lien on our records, the lienholder must apply for the duplicate title.<br />

You can also furnish our office with a statement of discharge with the vehicles<br />

description and a completed application for duplicate signed by the customer.<br />

The application must be signed and notarized. The top portion of the application (name,<br />

address, and vehicle or watercraft description) must always be completed. For sample<br />

form, see exhibit "B".<br />

Memorandum Certificate Application<br />

If the memorandum certificate is being applied for in conjunction with the issuance of a<br />

new title, the title number is not needed on this form. If you are applying for a<br />

memorandum on an existing title, furnish the title number (whenever possible) in the<br />

space provided.<br />

Please note that if a title is being issued as “tax-exempt to an out-of-state resident”, no<br />

memorandum certificate will be issued. The certificate is solely used to purchase <strong>Ohio</strong><br />

license plates and registrations.<br />

address, and vehicle or watercraft description) must be completed.<br />

Either the titled owner or the lienholder may apply for a memorandum certificate. The<br />

applicant’s signature must be notarized. For sample form, see exhibit "C".<br />

Salvage Title, Salvage Replacement and Salvage Duplicate Title<br />

Application<br />

Complete the required information at the top of the application (name, address, and<br />

vehicle description). In the case of a salvage replacement, insert the title number you are<br />

surrendering to our office.<br />

This form must be signed and notarized. For sample form, see exhibit "D".<br />

Other General Information<br />

Powers of Attorney<br />

A power of attorney is a legal document giving specific authority to a person to act on<br />

behalf of the person who gives this authority.<br />

The signature on any power of attorney should be the same as the signature on other<br />

papers required in the transaction. The forms are completed with the necessary<br />

information and are notarized. A minor cannot give someone power of attorney.<br />
