Applications for Title<br />

FRANKLIN COUNTY CLERK OF COURTS<br />

<strong>AUTO</strong> <strong>TITLE</strong> <strong>MANUAL</strong><br />

The title application is used to provide accurate and essential information in applying for<br />

the issuance of various types of titles. It is important that the information on this<br />

application be precise and legible.<br />

In all cases, regardless of type of title being applied for, the applicant’s name, address and<br />

social security number (or employer’s identification number, in the case of a company) as<br />

well as the vehicle or watercraft information must be completed on this form.<br />

When the evidence being presented is an <strong>Ohio</strong> title, the application is on the back of the<br />

certificate of title (the lower portion). This must be used in place of a separate form.<br />

Original Title Application<br />

This information will be used in applying for the issuance of the first <strong>Ohio</strong> title. The<br />

application must be accompanied by evidence of ownership. The evidence will usually<br />

be one of the following:<br />

Manufacturer’s Certificate of Origin<br />

Out-of-State Title<br />

Foreign Registration<br />

Court Order<br />

The NAME used on the application should be the applicant’s legal name including any<br />

epithet such as Jr., Sr., III, etc. In the case of an applicant who has no legal first or<br />

middle name, the name must read as follows:<br />

J. R. Doe (Initials Only)<br />

Abbreviations of names or nicknames are not acceptable. A married woman cannot<br />

apply as “Mrs. John R. Doe”; she must use her own given first name such as “Marie A.<br />

Doe”.<br />

A company, corporation, organization, etc. must use the full and legal name as it appears<br />

on the charter registered with the Secretary of State’s office.<br />

The SOCIAL SECURITY NUMBER or EMPLOYER’S IDENTIFICATION NUMBER (in<br />

the case of a company) must be entered on the application for title. All applicable social<br />

security numbers must be entered when two or more people are applying.<br />

The ADDRESS should be the complete, legal address of the individual or company,<br />

including the county of residence. In the case of an individual, this should be the address<br />

of his/her residence. A post office box is not acceptable for address purposes. Postal<br />

zone numbers are meant for post office use only and have no significance to <strong>Ohio</strong> title<br />

law.<br />

If there is to be a LIEN on the vehicle, fill in the lien holders complete name and address<br />

on the application. If there is not a lien, the word “none” must be stated.<br />
