<strong>AUTO</strong> <strong>TITLE</strong> <strong>MANUAL</strong><br />

The PURCHASE PRICE, TRADE-IN (if any), TAX BASE, GROSS TAX and, in the case<br />

of a sale by an <strong>Ohio</strong> dealer or vendor, the NET TAX PAID must be entered. Care should<br />

be taken to see that all figures are correctly entered. All taxable items should be included<br />

in the purchase price (extended warranty, luxury tax and documentary fees, etc.). All<br />

taxable items are listed in <strong>Ohio</strong> Dept. of Taxation section of this manual.<br />

If the individual or company is claiming a TAX EXEMPTION, check the appropriate box<br />

on the application and state the basis for exemption. A copy of the most recent tax<br />

directive is enclosed in this manual for your guidance.<br />

In the case of a dealer sale, enter the DEALER’S PERMIT NUMBER and VENDOR’S<br />

NUMBER. The permit number should be complete with any -01*, -02, -03, etc. that is<br />

needed to have the title reflect the name as the dealer wants it to appear. The asterisk<br />

indicates that the dealer would like the DBA and Corp. name printed on the title. In the<br />

case of a vehicle being sold to a leasing or rental company, this space on the application<br />

should be reserved for the leasing or rental company’s permit and/or vendor’s number.<br />

Please note that a rental company is not issued a dealer’s permit number. Only a<br />

vendor’s number is to be entered in this space for rentals.<br />

The APPLICANT’S SIGNATURE must be exactly as the name of the owner appears at the<br />

top of the application. When the legal name of the applicant consists of initials only, the<br />

legal name must appear after the applicant’s signature.<br />

When a title is being applied for in the name of an organization or company, the company<br />

name must appear with the signature of the individual acting on behalf of the company.<br />

The person signing on behalf of the company must state their position with the company<br />

or in what capacity they are signing, such as “title clerk”, “office manager”, “power of<br />

attorney”, etc. In the case of someone signing by “power of attorney”, the power of<br />

attorney form must accompany the paperwork to our office. For sample form, see<br />

exhibit "A".<br />

Replacement Title Application<br />

A title application is required in the instance when a title has been defaced during the<br />

execution of a casual sale. The application should be completed, signed by the titled<br />

owner and notarized.<br />

Duplicate Title Application<br />

In the event that an original title is lost, stolen or destroyed, any Clerk of Courts’ office<br />

can issue a duplicate title.<br />

The process is expedited if the original title number is provided on this application. If<br />

you do not have access to the original title number, you can call our office for this<br />

information.<br />
