<strong>RULES</strong> <strong>AND</strong> <strong>REGULATIONS</strong> <strong>FOR</strong> <strong>HOSPICE</strong> <strong>IN</strong> <strong>ARKANSAS</strong><br />

responsible for drugs and biologicals for patients, in so far as they are covered<br />

under the program and for ensuring that pharmaceutical services are provided in<br />

accordance with accepted professional principles and appropriate Federal, State<br />

and local laws.<br />

1. In facilities that obtains drugs and biologicals from community or institutional<br />

pharmacies:<br />

a) The Hospice shall have contractual arrangements to ensure services<br />

are available 24 hour 7 days a week to the patients in the Hospice<br />

facility.<br />

b) All prescription medications in the facility shall be patient specific and<br />

appropriately labeled.<br />

c) No prescription drug floor stock shall be allowed in the facility.<br />

2. The Hospice shall:<br />

a) Employ a licensed pharmacist; or<br />

b) Have a formal agreement with a licensed pharmacist to advise the<br />

hospice on ordering, storage, administration, disposal, and record<br />

keeping of drugs and biologicals; and<br />

c) Have a Pharmaceutical Service Committee which meets quarterly<br />

consisting of at least the Medical Director, Pharmacist, Nurse<br />

Manager, and Administrator. The committee shall be responsible for<br />

the following:<br />

1) Serve as an advisory group to the medical staff;<br />

2) Approve the policies and procedures for pharmaceutical<br />

service annually;<br />

3) Approve medication formulary annually;<br />

4) Approve floor stock annually; and<br />

5) Discuss medication errors and adverse drug reactions.<br />
