Dear Ms. Green,<br />

Pursuant to Rule 217 of the <strong>Kansas</strong> Supreme Court Rules Relating to the Discipline of<br />

Attorneys, I hereby voluntarily surrender my license to <strong>practice</strong> law in <strong>Kansas</strong>. Enclosed please<br />

find my original Supreme Court of <strong>Kansas</strong> license to <strong>practice</strong> law as well as my current annual<br />

attorney registration card.<br />

I am fully aware of Rule 217 and understand that by surrendering my license the Court will<br />

duly enter an order disbarring me and terminating all pending disciplinary actions. I understand<br />

that other jurisdictions will be notified of the Court’s action.<br />

I have read Rule 218 and have or will appropriately and timely comply with the Court’s<br />

directions contained therein.<br />
