Five maternal deaths (21.7%) were due to preventable causes <strong>of</strong> which two due to haemorrhage, two due to septicaemia and one due to<br />
toxaemia/hypertension. Pulmonary embolism was <strong>the</strong> main reported cause <strong>of</strong> death in eight cases (34.8%), and 7 women (30%) had<br />
underlying morbidities (four cases <strong>of</strong> heart failure/disease and one case <strong>of</strong> each sickle cell disease, colon cancer and bronchial asthma). Five<br />
women died <strong>of</strong> iatrogenic complications in <strong>the</strong> hospital (three cases <strong>of</strong> cardiac arrest and 2 <strong>of</strong> septic shock) two died <strong>of</strong> disseminated<br />
intravascular coagulation and one due to influenza A/H1N1v infection.<br />
