Also, Polk is one of an 8-county region that receives Ryan White grant funding from the state. In order to receive assistance, clients must be positive for HIV/AIDS, but must also provide evidence of an HIV/AIDS-related reason for assistance, such as: • Physician- authorized statement of need; • Discharge paperwork from a nursing home; and • Evidence of homelessness (hospital documentation of multiple emergency room visits, shelter data, etc.). The Health Dept. may provide a maximum of 21 weeks of housing assistance, but the client must provide evidence that he will be able to continue payments for housing after the 21 weeks (through employment, family member, etc.) Without a ‘plan’ for continued housing payments, they will not qualify for any housing assistance.
