PROVISIONAL LICENSE A provisional license is required for those under 18 years of age. DPID/STKR An address change sticker was issued to the holder of a disabled parking ID card. DPRK PERMIT A replacement disabled parking permit was issued. ID A replacement Identification card was issued. ID/STICKER Change of address sticker was issued to the holder of an identification card. NON ISSUE DMV received a written request for an address change. Although the record is updated with the new address, no sticker is issued because the person does not have a valid license or ID card. The license or ID card was expired, suspended, revoked, or canceled. MCIP A replacement motorcycle instruction permit was issued. STICKER An address change sticker was issued to an individual after the person notified DMV of a change of address. The sticker shows the new address and is affixed along the bottom portion of the license. SP A replacement student permit was issued. EP A replacement emergency operator’s permit was issued. Note: Stickers issued show updated address and are affixed along the bottom portion of the license, permit, or ID card. REISSUED LICENSES, PERMITS, OR IDENTIFICATION CARDS This entry is used when a driver license, permit, or identification card is reissued because the initial card was not received in the mail by the person. CDL ISSUANCE ENTRY COMMERCIAL DRIVER LICENSE (CDL) ENTRIES This entry is used when a CDL is issued, whether it is an original CDL issuance, a renewal, or a replacement (adding or deleting an endorsement or restriction is a replacement transaction). Prior to 2007, it was also used to record the license number of an out-of-state driver when DMV created an Oregon record due to a traffic conviction. RELIC Commercial licensed driver from another state was relicensed (non-CDL) in Oregon. (No longer used.) SURR License was surrendered. TO OR Record was transferred to Oregon from another state. TO OS Record was transferred to another state from Oregon. RET License is returned from another state. PLIC Previously licensed in another state. State identifier. Two-character abbreviation for Oregon or other jurisdiction involved. License number (for jurisdiction identified in field (f)). 11
