Several other entries may also appear on the driving record. The following pages explain the<br />

entries and how they will appear.<br />

IDENTIFICATION CARDS<br />

A person was issued an identification (ID) card. The card looks similar <strong>to</strong> the driver license, but<br />

does not grant any driving privileges.<br />

Identification card issued.<br />

Business date. This is the date the card was originally issued.<br />

Issue date. This is the date the ID card was last issued or renewed.<br />

Expiration date. This is the date the card expires.<br />

This field will indicate PARK ID if the person was issued a Disabled Placard Identification<br />

Card. These cards are issued only <strong>to</strong> people who do not possess a valid driver license or<br />

pho<strong>to</strong> ID card and who are physically disabled <strong>to</strong> the point that they cannot go <strong>to</strong> a field <strong>of</strong>fice<br />

for a pho<strong>to</strong> ID. When this entry appears on the record, it has a matching issuance entry for a<br />

disabled placard.<br />

COURT LIMITED LICENSE<br />

A judge limited a driver’s driving privileges at the time the driver appeared in court. DMV is required<br />

<strong>to</strong> enter the action on the driving record.<br />

Court has limited a driver’s privileges <strong>to</strong> operate a vehicle.<br />

Effective Date. Date the limitation takes effect.<br />

Expiration Date. Date the limitation expires.<br />

Court type and location. If the court name is more than five letters in length, it will be<br />

abbreviated. Court types that will display are: C = Circuit; D = District; M = Municipal; J =<br />

Justice; and X = Juvenile.<br />

Will display one <strong>of</strong> the following:<br />

BLANK Court’s docket number is more than six digits<br />

073652 Court’s docket number (Six digits or less)<br />

FILE NO: 1234 DMV’s internal file number<br />

DISABLED PERMITS<br />

A person has been issued a disabled parking permit which allows use <strong>of</strong> specifically marked<br />

parking spaces for disabled persons.<br />
