O/S RFHAZ* S Driver of a CMV transporting hazardous materials refused to submit to a blood
alcohol content test in another state. Suspension length is increased due to prior
major CDL/CMV offense in Oregon or an equivalent in another state. If driver has
other open implied consent suspensions on their record (includes any pending
suspension arising out of same arrest), this suspension begins on the ending date of
that suspension. Lifetime suspension of CDL only. Conditional reinstatement after 10
years (see OAR 735-070-0200).
O/SBACFAIL S Driver of a non-CMV failed a breath test in another state with a blood alcohol content
of .08% of more (any amount if under 21). Suspended until the driver is cleared in
the other state.
O/SVC CDL3 S Driver of a CMV violated an out-of-service order. Third or subsequent offense.
Suspended for five years. (This suspension is of the person’s CDL only.)
O/SVC HAZ S Driver of a commercial motor vehicle carrying hazardous materials, or a vehicle
designed to carry 16 or more passengers (including the driver), violated an out-ofservice
order. Suspended for one year. (This suspension is of the person’s CDL
only.)
O/SVC HAZ* S Driver of a commercial motor vehicle carrying hazardous materials, or a vehicle
designed to carry 16 or more passengers (including the driver), violated an out-ofservice
order. Second offense. Suspended for five years. (This suspension is of the
person’s CDL only.)
OS FAC CDL S DRIVER OF A CMV OR A DRIVER WITH A VALID CDL AT THE TIME OF OFFENSE FAILED TO APPEAR OR FAILED TO
COMPLY AS ORDERED BY ANOTHER STATE. SUSPENDED FOR FIVE YEARS OR UNTIL A CLEARANCE IS RECEIVED
FROM THE OTHER STATE, WHICHEVER OCCURS FIRST. (THIS SUSPENSION IS OF THE PERSON’S CDL ONLY.)
OSFAC CDL/ S Driver of a CMV or driver with a valid CDL at the time of offense failed to appear or
railed to comply as ordered by another state. Suspended for ten years or until a
clearance is received from the other state, whichever occurs first. (This suspension
is of the person’s CDL only.)

OWNER AC/H S After hearing suspension to OWNER ACC suspension.
OWNER ACC S A vehicle registered to one person and driven by someone else was involved in an
uninsured accident. Owner is suspended until they comply with future responsibility
filing requirements. Note: It is possible to see this suspension even if there is no
accident posted to the record.
PAR SIG C Withdrawal of parental signature on a license or permit for a driver under 18 years of
age. May reapply with a new parental signature, if declared an emancipated minor
by a court, reaches 18 years of age, or gets married.
PERJURY R Perjury or false affidavit to DMV under any law of this state requiring the registration
of vehicles or regulating their operation on highways. Revoked for one year.

pay child support. Suspended until a notice is received from a District Attorney or
Support Enforcement Division (SED) allowing a driver to reinstate. (This suspension
is of the person’s CDL only.)
