DWS/R CMV* S Driver of a CMV received a 2 nd conviction of DWS or DWR for violation committed
while operating a CMV. Suspension length is increased due to prior major CDL/CMV
offense in Oregon or an equivalent in another state. Lifetime suspension of CDL
only. Conditional reinstatement after 10 years (see OAR 735-070-0200).
DWS/R HAZ S Driver of a CMV transporting hazardous materials received a first conviction of
driving while suspended or revoked for violations committed while operating a CMV.
Suspended for three years. (This suspension is of the person’s CDL only and, for
suspensions effective 11/10/08 or later, is used for out-of-state drivers only.)
FAP21 DL S Suspended for making a false application for a license, permit or ID card. May be
eligible for early reinstatement if the sole reason for committing the offense was to
appear 21 or older for the purpose of purchasing or consuming alcohol or entering
premises prohibited to those under the age of 21. Suspended up to one year.
FAP21 DL/H S After hearing suspension to FAP21 DL suspension.

F INST IID S Driver has not installed Ignition Interlock Device (IID) when required after DUII
suspension. Suspended until driver installs (IID) or until ending date of IID
requirement (one year from end of DUII suspension for a first conviction or two years
for a 2 nd or subsequent DUII conviction).
F PAY TAX S Driver suspended in accordance with request from the Oregon Department of
Revenue, for failure to file a tax return or pay a tax. Suspended until a notice is
received from the Oregon Department of Revenue allowing driver to reinstate. (This
suspension is of the person’s CDL only.)
F PAY TX/H S After hearing suspension to F PAY TAX suspension.
F PER/DUTY S Failure to perform the duties of a driver involved in an accident. See FL/AT ELUD for
suspension lengths.

MEANS DMV DETERMINES THE SUSPENSION.
F USE IID C FAILURE TO USE IGNITION INTERLOCK DEVICE. SUSPENDED FOR SIX MONTHS OR UNTIL DEVICE INSTALLED,
WHICHEVER COMES FIRST.
F/A BAIL S DRIVER SURRENDERED DRIVER LICENSE TO A COURT FOR SECURITY UNTIL COURT APPEARANCE DATE AND THEN
FAILED TO APPEAR IN COURT. SUSPENDED UNTIL THE COURT SUBMITS A CLEARANCE OR FOR FIVE YEARS,
WHICHEVER COMES FIRST.
F/A BAIL/ S Driver surrendered driver license to a court for security until court appearance date
and then failed to appear in court. Suspended until the court submits a clearance or
for ten years, whichever comes first.
F/A DUII S FAILURE TO APPEAR IN COURT FOR A DUII CITATION. SUSPENDED UNTIL THE COURT SUBMITS A CLEARANCE OR
FOR FIVE YEARS, WHICHEVER COMES FIRST.
F/A DWR S FAILURE TO APPEAR AT A COURT HEARING FOR DRIVING WHILE REVOKED CITATION. DRIVING PRIVILEGES ARE
SUSPENDED FOR FIVE YEARS OR UNTIL THE DRIVER PROVIDES A CLEARANCE FROM THE COURT.
F/A DWR HO S FAILURE TO APPEAR IN COURT FOR A DRIVING WHILE A HABITUAL OFFENDER CITATION. SUSPENDED FOR FIVE
YEARS OR UNITIL COURT RECOMMENDS REINSTATEMENT.
F/A ELUD S FAILURE TO APPEAR IN COURT FOR FLEEING OR ATTEMPTING TO ELUDE A POLICE OFFICER. SUSPENDED UNTIL
THE COURT SUBMITS A CLEARANCE OR FOR FIVE YEARS, WHICHEVER COMES FIRST.
F/A FPDD S FAILURE TO APPEAR IN COURT FOR FAILURE TO PERFORM THE DUTIES OF A DRIVER (NON-INJURY). SUSPENDED
UNTIL THE COURT SUBMITS A CLEARANCE OR FOR FIVE YEARS, WHICHEVER COMES FIRST.
