N/ENT ID C Not entitled <strong>to</strong> an identification card. An error may have been made when card was<br />

issued; a business address may have been used rather than a residence address;<br />

the person may not be an <strong>Oregon</strong> resident, etc. Canceled until the driver meets<br />

requirements.<br />

NEG HOM/IN R DRIVER CONVICTED OF NEGLIGENT HOMICIDE RESULTING FROM THE OPERATION OF A MOTOR VEHICLE.<br />

SENTENCING INCLUDED INCARCERATION. THE REVOCATION IS IMPOSED UPON NOTICE OF THE CONVICTION, BUT<br />

HAS NO ENDING DATE UNTIL DMV RECEIVES NOTICE OF THE PERSON’S RELEASE FROM THE INCARCERATION<br />

IMPOSE BY THE RELATED CONVICTION. THE REVOCATION PERIOD ENDS EIGHT YEARS FROM THE DATE OF<br />

RELEASE.<br />

NEG HOMC/P R Driver convicted <strong>of</strong> criminally negligent homicide resulting from the operation <strong>of</strong> a<br />

mo<strong>to</strong>r vehicle. Revoked permanently by court order. Person may petition the court <strong>to</strong><br />

res<strong>to</strong>re driving privileges no sooner than ten years from the date the person is<br />

released from incarceration.<br />

NEG HOMICD R Driver was convicted <strong>of</strong> negligent homicide resulting from the operation <strong>of</strong> a mo<strong>to</strong>r<br />

vehicle. Revoked for ten years. If the person was incarcerated as a result <strong>of</strong> the<br />

conviction, the revocation period will not begin until DMV receives notice that they<br />

have been released from incarceration. Some older entries will appear on records<br />

showing a five or eight year revocation period.<br />

NEGHOM/INC R Driver convicted <strong>of</strong> criminally negligent homicide resulting from the operation <strong>of</strong> a<br />

mo<strong>to</strong>r vehicle. Sentencing included incarceration. The revocation is imposed upon<br />

notice <strong>of</strong> conviction, but has no end date until DMV receives notice <strong>of</strong> the person’s<br />

release from incarceration for this and any other crime arising from the same criminal<br />

episode. The revocation period ends ten years from the date <strong>of</strong> release from<br />

incarceration.<br />

NSF CHECK S A check returned <strong>to</strong> DMV for non-sufficient funds and the person has not reimbursed<br />

DMV. Suspended indefinitely or until payment is made, including an NSF check fee.<br />

O/S BFLCMV S Driver <strong>of</strong> a CMV failed breath test in another state with a blood alcohol content <strong>of</strong><br />

.04% or greater. Suspended for one year. (This suspension is <strong>of</strong> the person’s CDL<br />

only.)<br />

O/SBFLCMV* S Driver <strong>of</strong> a CMV failed a blood test in another state. Suspension length is increased<br />

due <strong>to</strong> prior major CDL/CMV <strong>of</strong>fense in <strong>Oregon</strong> or an equivalent prior <strong>of</strong>fense in<br />

another state. Lifetime suspension <strong>of</strong> CDL only. Conditional reinstatement after 10<br />

years (see OAR 735-070-0200).<br />

O/SBFLHAZ S Driver <strong>of</strong> a CMV transporting hazardous materials failed breath test in another state<br />

with a blood alcohol content <strong>of</strong> .04% or greater. Suspended for three years. (This<br />

suspension is <strong>of</strong> the person’s CDL only.)<br />

O/S BFLHAZ* S Driver <strong>of</strong> a CMV or driver with a valid CDL at the time <strong>of</strong> <strong>of</strong>fense transporting<br />

hazardous materials failed a blood test in another state. Suspension length is<br />

increased due <strong>to</strong> prior major CDL/CMV <strong>of</strong>fense in <strong>Oregon</strong> or an equivalent prior<br />

<strong>of</strong>fense in another state. Lifetime suspension <strong>of</strong> CDL only. Conditional reinstatement<br />

after 10 years (see OAR 735-070-0200).<br />

O/S REFUSE S Driver <strong>of</strong> a non-CMV refused a chemical breath test in another state. Suspended<br />

until the driver is cleared in the other state.<br />

O/S RF CMV S Driver <strong>of</strong> a CMV or a driver holding a CDL at the time <strong>of</strong> the <strong>of</strong>fense refused <strong>to</strong><br />

submit <strong>to</strong> blood alcohol content test in another state. Suspended for three years.<br />

(This suspension is <strong>of</strong> the person’s CDL only.)<br />

O/S RF HAZ S Driver <strong>of</strong> a CMV transporting hazardous materials refused <strong>to</strong> submit <strong>to</strong> a blood<br />

alcohol content test in another state. Suspended for five years. (This suspension is<br />

<strong>of</strong> the person’s CDL only.)<br />

O/S RFCDL* S Driver <strong>of</strong> a CMV or a driver with a valid CDL at the time <strong>of</strong> <strong>of</strong>fense refused <strong>to</strong> submit<br />

<strong>to</strong> blood alcohol content test in another state. Suspension length is increased due <strong>to</strong><br />

prior major CDL/CMV <strong>of</strong>fense in <strong>Oregon</strong> or an equivalent prior <strong>of</strong>fense in another<br />

state. Lifetime suspension <strong>of</strong> CDL only. Conditional reinstatement after 10 years (see<br />
