Good discharge planning is crucial <str<strong>on</strong>g>for</str<strong>on</strong>g> successful<br />

reintegrati<strong>on</strong> into <strong>the</strong> community as well as effective<br />

and efficient use of limited hospital resources.<br />

While it is known that <strong>the</strong> transfer of resp<strong>on</strong>sibility <str<strong>on</strong>g>for</str<strong>on</strong>g><br />

management from inpatient to <strong>the</strong> community can be<br />

difficult, insufficient attenti<strong>on</strong> and resources are often<br />
