Revisions to Form and standards adapted<br />
1 st Quarter Report: Goal 2, Steps 1 - 4: A workgroup is being formed to address changes needed to improve the Supervisory Review<br />
Form and implementation process. The committee is aware <strong>of</strong> PIP requirements to place siblings together, if possible or feasible. They<br />
expect to revise the Supervisory Review Form to document efforts to place siblings together.<br />
2 nd Quarter Report - No report for this quarter.<br />
3 rd Quarter Report: A work group, consisting <strong>of</strong> social services pr<strong>of</strong>essionals, is reviewing the Supervisory Review Form to assure it<br />
properly measures specified outcomes for service delivery. This team <strong>of</strong> pr<strong>of</strong>essionals provides a broad array <strong>of</strong> expertise in child<br />
welfare policy, procedures and practice issues. The review form is being revised in accordance with state policy mandates. One <strong>of</strong> the<br />
mandates requires documentation <strong>of</strong> efforts to place siblings together.<br />
4 th Quarter Report: Partially Achieved. Efforts to revise the Supervisory Review Form and standards continue. Revisions include<br />
documentation <strong>of</strong> efforts to place siblings together.<br />
5th Quarter Report: Partially Achieved. The state has made the decision to discontinue using the Supervisory Review Guide and<br />
replaces it with the Child Placement <strong>Services</strong> E & R Social <strong>Services</strong> Review Guide, developed by the Evaluation and Reporting (E &<br />
R) Section. This instrument is already developed and available to the staff. Also, it incorporates PIP requirements for Georgia. It<br />
includes a reference to efforts to place siblings together (under Item 28). According to the first round <strong>of</strong> QCRs efforts to place siblings<br />
together was documented in 10 <strong>of</strong> the 13 applicable cases. See the attached Qualitative Review Report for October 2003 --<br />
December2003.<br />
