When PVT is compared with standard community treatment, there are<br />

no differences <strong>in</strong> the programme f<strong>in</strong>alisation rate (n = 284; RR = 0.95; 95%<br />

CI between 0.52 and 1.7) 165 .<br />

Despite f<strong>in</strong>d<strong>in</strong>g a lower ratio of hospital readmission among patients<br />

who received PVT (n = 887; RR = 0.79; 95% CI between 0.65 and 0.95),<br />

this difference was no longer significant when a random effect model was<br />

used due to the high heterogeneity <strong>in</strong> 3 studies (n = 887; RR = 0.71; 95%<br />

CI between 0.48 and 1.04) 165 , so the evidence is <strong>in</strong>sufficient to determ<strong>in</strong>e if<br />

there are significant differences between PVT and ST <strong>in</strong> hospital readmission<br />

ratios.<br />

Pre-vocational Tra<strong>in</strong><strong>in</strong>g (PVT) vs. PVT + alternatives (clubhouse, economic<br />

re<strong>in</strong><strong>for</strong>cement, psychological <strong>in</strong>tervention, transitional employment)<br />

In the work by Beard et al 170 no significant differences have been found between<br />

the PVT accord<strong>in</strong>g to the clubhouse model (device where and from<br />

where they offer: a) social activities, b) daily job <strong>in</strong>structions <strong>in</strong> teams, c)<br />

transitional employment <strong>for</strong> 3-4 months until phases a and b are completed,<br />

and placement <strong>in</strong> a job with adaptation and support) compared with standard<br />

community treatment, with respect to obta<strong>in</strong><strong>in</strong>g competitive employment<br />

and any other employment variable, with a 3 to 12 month follow-up (n =<br />

215; RR = 0.95; 95% CI: between 0.77 and 1.17).<br />

There are less readmissions <strong>in</strong> the PVT-clubhouse group, compared<br />

with the standard community treatment ( n = 215; RR = 0.69; 95% CI: between<br />

0.46 and 0.96) 170 .<br />

The 6-month study by Bell et al 176 , a paid part-time sheltered job <strong>in</strong>tervention,<br />

<strong>in</strong> the context of a hospital, from caretakers to adm<strong>in</strong>istrative staff,<br />

with 50-m<strong>in</strong>ute weekly group support, shows a larger number of people <strong>in</strong><br />

any type of employment <strong>in</strong> the PVT group + alternatives (economic re<strong>in</strong><strong>for</strong>cement),<br />

compared with just PVT (n = 150; RR = 0.40; 95% CI: between<br />

0.28 and 0.57), and they also earned significantly more money a month<br />

($192 and $32 respectively; t = 7.56; p = 0.0001). The competitive work was<br />

not a result studied.<br />

The participants <strong>in</strong> the paid sheltered work group presented greater permanence<br />

(n = 150; RR = 0.53; 95% CI: between 0.39 and 0.71; NNT = 3),<br />

less number of hospital readmissions (n = 150; RR = 0.55; 95% CI between<br />

0.31 and 0.96; NNT = 6) and less scores <strong>in</strong> symptoms (measured on the<br />

PANSS scale; p < 0.02) than the PVT group 176 .<br />

In the studies by Kl<strong>in</strong>e & Hois<strong>in</strong>gton 177 , and Blankerz & Rob<strong>in</strong>son 178 ,<br />

PVT and psychological techniques were carried out. In the study by Kl<strong>in</strong>e<br />

& Hois<strong>in</strong>gton 177 there is a 1.5 hour group <strong>in</strong>tervention <strong>for</strong> 12 weeks, when<br />

discussions are held on labour values and which is aimed at reduc<strong>in</strong>g anxiety.<br />

In the Blankerz & Rob<strong>in</strong>son study 178 counsell<strong>in</strong>g is applied as well as<br />

social technique learn<strong>in</strong>g and group sessions with re<strong>in</strong><strong>for</strong>cement by partial<br />

objectives. Both studies presented favourable results with respect to obta<strong>in</strong><strong>in</strong>g<br />

competitive employment (n = 142; RR = 0.86; 95% CI between 0.78 and 0.95;<br />

NNT = 7); but heterogeneity was observed <strong>in</strong> the studies (p = 0.007). When<br />

the re-analysis was per<strong>for</strong>med with a random effects model, no significant<br />

differences were found (n = 142; RR = 0.76; 95% CI between 0.44 and 1.33).<br />
