users and can be accessed anywhere, at any time, with proper authorization. This includes
documentation entered directly online and those components of the record where there is direct
integration between systems, such as laboratory and radiology information systems.
Components of the record where the caregiver documents on paper or in secondary systems not
interfaced with the CIS, the documentation is stored in hard copy in the paper medical record in
HIM. Various components of the Clinical Information System have been implemented in most
locations in the MMC network. As more elements of the medical record become part of CIS,
more information will be available on line and less in hard copy.
DEPARTMENT SERVICES
The Health Information Management (HIM) Department provides 24- hour/ 7 days-per-week
medical record services at both the Moses and Weiler Divisions.

Retrieval of Medical Records:
For Patient Care: Records needed for immediate patient care can be retrieved 24 hours a day.
The following information is required:
• Valid MMC Identification Card
• Patient Name
• Medical Record Number
• Requester Name/location/telephone number
• Date needed and purpose.
• For Research, Review and other purposes: Advance notice is required for retrieval of records
for reasons other than direct patient care. House staff must show approval of research project
by the supervising attending physician.
• For a data search, for research projects, call ext. 2208 at Moses and 2747 at Einstein
• To retrieve records in bulk discuss volume, time frames, etc with File Room Supervisor or
Manager at the appropriate site.
