available. Materials are printed on yellow paper and distributed to Moses and Weiler in/outpatient
areas, including satellites.
A newsletter called "UPDATE" listing over 1,200 patient education sheets can be found on
Montefiore’s Intranet site. New and revised publications are highlighted. Materials may be
ordered in quantity. Topics not listed can be obtained by patients and staff from the department's
library, resource files and Internet searches between 9:00 AM - 5:00 PM. A listing of Support
Groups sponsored throughout the Medical Center, to provide assistance to out-patients/families
and the community, is available upon request. Health Education video programs are shown on
in-patient television sets and in some out patient areas, free-of-charge. A television schedule is
distributed to patients.

The mission of the MMC Patient Safety Program is to continuously improve care systems in
order to minimize the risks posed to patients as a result of exposure to the healthcare system. As
such, the mission of the program is well aligned with MMC’s mission of excellence in patient
care and the MMC strategy of improving organizational performance.
Tenets of the program include:
• Achieving improvement through an emphasis on processes and systems of care over the
identification of individuals
• Drawing upon network-wide surveillance mechanisms as information resources relating
to patient safety
• Enlisting multidisciplinary participation in improving organizational performance
• Learning from adverse events as well as medical errors that may or may not have resulted
in harm
• Valuing patient, family, and staff feedback regarding patient safety
PATIENT SAFETY OFFICER AND COMMITTEE
The primary role of the Patient Safety Officer is to facilitate and enable institutional patient
safety activities and the institutional Patient Safety Program. The Patient Safety Officer reports
to the Vice President, Senior Medical Director.
The Patient Safety Committee serves as an organizational structure to facilitate and enable
patient safety activities and manage the Patient Safety Program. The Patient Safety Committee
is accountable to the Montefiore Quality Council. The Patient Safety Program cannot lead all
activities that relate to improving patient safety at MMC and is not meant to substitute for the
existing institutional activities and structures noted earlier, but to complement them, by focusing
on patient safety related compliance, coordination, and communication.
