18. Patients at Risk from Self-Poisoning If a Healthcare Professional on a Community visit considers that a patient is at risk and capable of planned or accidental self-poisoning with their prescribed medication they should:- • Remove the medicines from the home with the patient’s or relative’s permission, and store in a locked cupboard at team base. The medicines removed must be recorded in the patient’s records. • Document reasons for decision and notify Psychiatrist and/or GP. • Enter details of visit, reason for removal, medicine, strength and quantity in the patient’s nursing record. • If there are mental health capacity issues then the prescriber must be contacted for further discussion If the situation does not warrant immediate removal but the healthcare professional has a concern these must be documented and shared with the rest of the team and prescriber. If a patient being discharged or given leave from hospital is similarly at risk, they should only be provided with a small amount of medicine, exact quantities can be discussed with the prescriber. 44
19. Compliance aids All compliance aids should be filled by <strong>Dudley</strong> Group of Hospitals Pharmacy Department or by a community pharmacist after prior agreement with the supplier. <strong>Medicines</strong> quality and effectiveness can be affected by decanting into these aids and pharmaceutical advice is essential before one can be supplied. 45
