Provides primary medical care for low-income, uninsured and underserved families in <strong>Kitsap</strong> County. May assist established PCHS<br />
patients with prescription purchases or samples. Accepts DSHS, Healthy Options, Basic Health and other insurances. Patients charged on<br />
a sliding scale based on income. Participant needs to call in the morning to schedule an appointment. Also provides dental, mental health<br />
care and WIC nutrition support service. Bremerton Clinic now open extended hours--week nights and weekends. Dental services for<br />
children only unless it is an emergency and then only if there is an opening.<br />
3M PHRMEACEUTICALS-PATIENT ASSISTANCE PROGRAMS<br />

To provide 3m pharmaceuticals prescription medications to those individuals who are not covered by a third-party prescription insurance<br />
plan, do not qualify for government assistance, and who income and medical expenses meet the qualification of the 3M program.<br />
