9.13 Medical Providers for Work-Related Injuries<br />
Unless you have completed a Pre-Designated physician form found at:<br />
www.ehs.uci.edu/apps/hr/index.jsp<br />
UCI University’s medical providers for work related injuries are listed below.<br />
* For Non-Life Threatening Work-Related Injuries<br />
Follow the procedures below:<br />
Campus: UCI Student Health during work hours and Newport Urgent Care off hours (walk-ins OK)<br />
UCIMC: May see any UCI provider, consult the GSHIP program.<br />
After hours: may see any UCI provider; call Referral Center 714-456-7890<br />
Offsite location for after hours: UCIMC or as directed by referral center<br />

• UCI Undergraduate students (non-employees)<br />
ALL: Refer to Student Health Services on campus 824-5304<br />
After Hours: Consult the Student Health services Website: http://www.shs.uci.edu/em,ergency.html.<br />
• UCI Employees or Student Employees (both graduate and undergraduate)<br />
During regular business hours: Contact workers Compensation 949-824-7008<br />
1. Treatment may be provided at one of the following clinics by having your Supervisor or Department<br />
Administrator contact the clinic directly:<br />
o Newport Urgent Care, Newport Beach<br />

o Occupational Health Clinic, Orange<br />
(714) 456-8300 (at UCI Medical Center, map)<br />
o ProCare Work Injury Center, Irvine<br />
(949) 752-1111 (located off campus, map)<br />
o Kaiser Occupational Health Center, Santa Ana<br />
(714) 830-6660 (located off campus, map)<br />
o East Edinger Urgent Care, Santa Ana<br />
(714) 541-8464 (located off campus, map)<br />
o Occupational Services, Long Beach<br />
(562) 933-0085 (located at Long Beach Memorial Hospital, map)<br />
2. If you are unable to seek medical attention at one of these clinics, contact the Workers' Compensation<br />
Unit, at (949) 824-9152, for clinic referral.<br />
In the event a Supervisor or Department Administrator is not available go directly to the clinic.<br />
• For Life-Threatening Emergency Care<br />
For immediate life threatening injuries call 911. If poisoning is suspected, contact the poison Control Center at<br />
800-222-1222.<br />
