Quality systems will also be developed to ensure accurate recording of patient<br />

information and details to ensure care continuity, as our vulnerable<br />

populations are at risk of having their information inaccurately recorded which<br />

compromises their health care. Having the medical centre as the central place<br />

for recording of this information with the agreement that other social support<br />

and NGOs information relevant to the health care for patients is shared.<br />

<strong>Community</strong> Engagement<br />

Effective community engagement with internal and external groups and<br />

organisations, relationships with youth services and elderly care services, and<br />

other social services that care for our vulnerable populations, further enhances<br />

the provision and facilitation of care for these groups.<br />

Cultural Responsiveness<br />

Cultural expertise, training and resources will be available for staff to increase<br />

their understanding of important values and protocols relating to the cultures<br />

of vulnerable populations. Implementing Te Arawhata Totika, the cultural<br />

competency framework, will provide the foundation for this work in<br />

<strong>Wairarapa</strong>.<br />
