Section II: Moderate Rehabilitation Projectsowner with either a letter of support that indicates the PHA’s consent or a letter that declines theowner’s request describing the reasons the PHA is declining the request.E. Submission of Request to HUD HeadquartersOnce the owner obtains the administering PHA’s and eligible households’ consent toretroactively convert EV assistance to PBV assistance, the owner must submit to <strong>RAD</strong>@hud.govits request, including:1. A certification from the PHA that the applicant has met all requirements described inparagraphs A through D above;2. A Financing Plan. Please see Attachment 1A.1 and 1A.2 for Financing Plan submissionrequirements and Feasibility Benchmarks. (Where the term ―PHA‖ is used to refer to thePHA as owner, the term ―owner‖ applies to Mod Rehab applicants.) HUD will releaseseparate guidance detailing what elements of the Financing Plan may be waived forprojects using FHA insurance; and3. Any other information necessary to evaluate the application that HUD may request at itsdiscretion.F. Approval and Execution of PBV HAP or Agreement to Enter into a Housing AssistancePayment (AHAP) ContractWithin 60 days of the owner’s submission, HUD Headquarters will review the Mod Rehabowner’s request, make a determination of whether the conversion of assistance is approved, andnotify the administering PHAThe PHA will verify the number of units that are eligible to receive PBV assistance under theconversion, and will perform income re-examinations on households whose incomes have notbeen re-determined within 12 months of the owner’s approved application. The owner is advisedthat if households do not meet income requirements, those units will not be assisted under thePBV program. Therefore, the total number of units assisted under the PBV HAP contract may befewer than originally requested by the owner.The PHA will also inspect the converting units to determine that they meet HQS, as required byPBV program rules. Units must meet HQS prior to execution of the PBV HAP Contract. If theunits do not meet the definition of existing housing under the PBV program, the PHA and ownermust enter into an AHAP and all rehabilitation must be performed pursuant to the terms of theAHAP.<strong>PIH</strong>-<strong>2012</strong>-<strong>32</strong> (HA) Rental Assistance Demonstration – Final Implementation 114
