Appendix F. N. L. R. B. Rules and Regulations 223the <strong>Board</strong> or the Regional Director approves the withdrawal of anypetition the case shall be closed. Except as provided in Section 11of this Article, such petition shall be filed with the Regional Directorfor the Region wherein the contemplated bargaining unit exists, or, ifthe contemplated bargaining unit exists in two or more Regions, withthe Regional Director for any of such Regions. Such petition shall bein writing, the original being signed and sworn to before any notarypublic or other person duly authorized by law to administer oathsand take acknowledgments or any agent of the <strong>Board</strong> authorized toadminister oaths or acknowledgments. Three copies of the petitionshall be filed.'SEC. 2. Same; contents.—(a) Such petition, when filed by anemployee or any person or labor organization acting on behalf ofemployees, shall contain the following:(1) The name and address of the petitioner.(2) The name and address of the employer or employers involvedand the general nature of their business.(3) A brief statement setting forth the nature of the questionaffecting commerce that has arisen concerning the representationof employees.(4) Any other relevant facts.(b) Such petition, when filed by an employer shall contain thefollowing:(1) The name and address of the petitioner.(2) The general nature of petitioner's business.(3) A brief statment setting forth that a question or controversyaffecting commerce has arisen concerning the representationof employees in that two or more labor organizations have presentedto the employer conflicting claims that each representsa majority of the employees in the unit or units claimed to beappropriate.(4) Any other relevant facts.SEC. 3. Same; investigation by Regional Director; definition ofparties; Notice of Hearing; service of notice; withdrawal of notice.—After a petition has been filed, if it appears to the Regional Directorthat an investigation should be instituted he shall institute such'Blank forms for filing such petitions will be supplied by the Regional Director upon request.
