The pro<strong>for</strong>ma should be used both as a checklistto ensure that all the necessary stages of therebanding process have been adhered to, and as arecord of the process <strong>for</strong> payroll purposes.2. Column headings are to be interpreted as:- Stage: a step in the process which must becompleted- Evidence required: documentation/data/inputthat must be available in order to facilitate adecision at the relevant stage- Documentation: the <strong>for</strong>mal confirmation thatthe stage has been followed through tosuccessful completion.3. In the pro<strong>for</strong>ma, references to the action teamshould be taken to refer to the Regional Improvingjunior doctors working lives action team or anysuccessor body.4. Where a decision from the Action Team isindicated, such a decision must be agreed by at aminimum, both a junior doctor employee and aBMA junior doctor representative, and will becoordinated by an officer acting with the fullauthority of, and nominated by the Action TeamChair.6. In recognition of the range of different monitoringprocesses used in the regions and not wishingeither to duplicate current practices or to create anunnecessary burden on trusts we do not proposeto be prescriptive in the way supportingmonitoring data is to be presented. However:- evidence of monitoring must con<strong>for</strong>m to therequirements of the documentation issued asguidance accompanying HSC 2000/031- monitoring and/or analysis data produced bysome software packages such as ND2000 willbe acceptable <strong>for</strong> the purpose of this exercise –further guidance will be issued in due course.7. Where provisional banding is authorisedmonitoring should take place within six weeks ofthe implementation of new workingarrangements, and all necessary actions taken toensure that the results of the monitoring arereflected in banding and salary.5. The order of the stages in the Pro<strong>for</strong>ma does notfollow the order stated in AL(MD)1/01; this is tofollow a logical process. It would <strong>for</strong> example beappropriate in most cases <strong>for</strong> the Action Team todiscuss and agree revised arrangements withjuniors and their employers in advance of seekingeducational approval.63
