<strong>DAP</strong> <strong>2009</strong>/<strong>10</strong> <strong>Final</strong>DHB Local Activity for the National Quality Improvement Programme1 Optimising the Patient Journeya) The DHB will continue to actively participate in the national collaborative processes.b) Progress our own patient journey related initiatives: delivery of acute care, maternityservices and oral health.2 Management of <strong>Health</strong>care Incidentsa) The DHB will continue to actively participate in the national collaborative processes.b) Ensure that our DHB Reportable Events Policy is in line with the national Managementof <strong>Health</strong>care Incidents Policy.c) The DHB will continue to market/promote the national programme across the DHBd) The DHB will release staff for the comprehensive training programme on incidentmanagement that is scheduled for July <strong>2009</strong>.3 Infection Prevention and Controla) The DHB will continue with the implementation of the Hand Hygiene Project Stage 2Rollout.b) The DHB will continue to participate in the national learning sessions with DHBs toeducate local teams on the guidelines, the implementation strategy, improvementmethod and achieving change.c) The DHB will implement the infection prevention and control strategies as theybecome available4 National Mortality Review Systemsa) The DHB will continue to work with the National CEO Group (via the Lead CEO) andthe Ministry of <strong>Health</strong> to assist other DHB’s in the establishment of a local Child andYouth Mortality Review CommitteeDHB Local Activity for the National Quality Improvement Programme5 Safe Medicines Managementa) The DHB will continue to actively participate in the applicable working groups.b) The DHB will consider becoming a pilot site for the medication reconciliation project.c) The DHB will implement the medication safety project outcomes and strategies asthey become available.21
