PHS SF424 (R&R) <strong>Individual</strong> <strong>Fellowship</strong> <strong>Application</strong> <strong>Guide</strong>Specific <strong>Application</strong>: Post ReviewTelephone or email the NIH or AHRQ Program Official named in the Summary Statement for theapplication.1.7 AuthorizationNIH and AHRQ request the information described in these instructions pursuant to the statutoryauthorities contained in Section 487 <strong>of</strong> the PHS Act, as amended (42 USC 288). Therefore, suchinformation must be submitted if an application is to receive due consideration for an award. Lack <strong>of</strong>sufficient information may hinder the ability to review an application and to monitor the awardee'sperformance.The statutory authorities for the <strong>Fellowship</strong> programs are contained in the following:F30, F31, F32, and F33 Authority: Sections 301(a) and 487 <strong>of</strong> the PHS Act, as amended (42 USC 241aand 42 USC 288), 42 CFR Part 66.F05 Authority: Section 307, 42USC 2421 and 42 CFR Part 63a.F37 Authority: Section 472, 42 USC 286b-3 and 42 CFR Part 61.AHRQ Authority: Section 487,Sections 304, 902, and 935 <strong>of</strong> the PHS Act, 42 USC 242b, 299, and 299c-4 and 42 CFR 67, Subpart A.1.7.1 Collection <strong>of</strong> Personal Demographic DataFederal agencies have a continuing commitment to monitor the operations <strong>of</strong> its review and awardprocesses to detect, and deal appropriately with, any instances <strong>of</strong> real or apparent inequities. In addition,Section 403 <strong>of</strong> the 2007 NIH Reform Act requires NIH to report to Congress specifically on postdoctoralindividuals supported on research grants, and section 489 <strong>of</strong> the PHS Act requires NIH to perform acontinuing assessment <strong>of</strong> research personnel needs. Personal demographic data on PD/PIs and those witha postdoctoral role is vital to comply with these requirements.NIH and AHRQ collect personal data through the eRA Commons Personal Pr<strong>of</strong>ile. The data is providedone-time by the individual through a secure, electronic system, is confidential, and is maintained underthe Privacy Act record system 09-25-0036, “Grants: IMPAC (Grant/Contract Information).” Whencompleting the data entry in the Commons Personal Pr<strong>of</strong>ile, the individual is responsible for providingtrue, accurate, and complete data. All analyses conducted on date <strong>of</strong> birth, citizenship, gender, race,ethnicity, disability, and/or disadvantaged background data will report aggregate statistical findings onlyand will not identify individuals. Declining to provide information does not affect consideration <strong>of</strong> anapplication, however, for some applications (e.g., Kirschstein-NRSA and research career developmentawards) citizenship is required to determine eligibility.The PHS also requests the last four digits <strong>of</strong> the Social Security Number (SSN) for accurate identification<strong>of</strong> individuals and for management <strong>of</strong> PHS grant programs. Please be aware that no individual will bedenied any right, benefit, or privilege provided by law because <strong>of</strong> refusal to disclose this portion <strong>of</strong> theSSN. The PHS requests the last four digits <strong>of</strong> the SSN under Sections 301(a) and 487 <strong>of</strong> the PHS Act asamended (42 U.S.C. 241a and U.S.C. 288).1.8 Paperwork BurdenThe PHS estimates that it will take approximately 18 hours to complete this application for an individualfellowship. This estimate excludes time for development <strong>of</strong> the research training plan. Other items such ashuman subjects are cleared and accounted for separately and therefore are not part <strong>of</strong> the time estimate.Part I: Instructions for Preparing and Submitting an <strong>Application</strong> I-8
