HEALTH INSURANCETijuana Flats’ health insurance plan provides employees and their dependents access to medicalinsurance benefits.Eligible employees may participate in the health insurance plan subject to all terms and conditions of theagreement between Tijuana Flats and the insurance carrier.A change in employment classification that would result in loss of eligibility to participate in the healthinsurance plan may qualify an employee for benefits continuation under the Consolidated OmnibusBudget Reconciliation Act (COBRA). Refer to the Benefits Continuation (COBRA) policy for moreinformation.Details of the health insurance plan are described in the Summary Plan Description (SPD). A SPD andinformation on cost of coverage will be provided in advance of enrollment to eligible employees. RELOCATION POLICY Eligibility: all salaried exempt employees being reassigned to work for locations greater than 50miles away from their former work assignment will be eligible for relocation assistance. Limitedrelocation assistance may be provided on a case-by-case basis to nonexempt associates. Provisions ofsuch agreement will be contingent upon approval from the COO and the HR Department. Duration: any assistance provided under the terms of this procedure will be provided within sixmonths of the date the associate is reassigned to the new location. Advances/Reimbursement: Some assistance expenses will be handled via corporateadvance while others will be handled as a reimbursement item similar to business travel expenses.These will be identified in the associate’s relocation agreement. Repayment Provision: if the employee elects to voluntarily leave the company within anallotted time (1 year) from the move, the associate is expected to reimburse the company for costsincurred. Details will be stated in the agreement. Taxes: the company will maintain a record of cumulative relocation expenses and will provide theappropriate tax forms to the employee for personal federal and state income tax filings in theappropriate year. Agreement: each relocating employee will be provided with a Relocation Agreement that will setforth the specific obligations to both the company and the employee. Where possible, suchagreements will be approved and provided to the associate (if possible) 10 days in advance of therelocation. Categories of Approved Relocation Expenses and Limitations: unlessexpressly approved in advance by the COO and the HR department. The following categories ofexpenses will be provided under this procedure:1. Pre-Assignment site visit / miscellaneous expenses: .Tijuana Flats will provide $500.00plus $100.00 for each additional family member. Maximum $800.00 in advance for the followingtype of expenses: Transportation to visit the local area for a period of two days, lodging and mealsduring this visit and misc. moving expenses. This visit and your schedule must be pre-approved bythe HR Department2. Relocation of household goods: The employee will be responsible for obtaining a Rental truckand must keep receipts for the truck rental, packing supplies, hand truck and gas forreimbursement. Tijuana Flats will reimburse up to $500.00 for Truck and Rental items. Must havereceipts for reimbursement.3. Lease break if necessary: The company will provide assistance for costs incurred with themove, including lease breakage, and loss of deposits for early termination.
