252 จิรายุ เอื้อวรากุลรอยด36 ยา อื่น ๆ ที่ มี การ ทดลอง ใช ไดแก Nafmostatmesilate 37 และ chloroquine ซึ่ง เปน serine proteaseinhibitor ที่ใชยับยั้ง cytotoxic T- cells หรือ daclizumab ซึ่ง เปน humanized anti-interleukin-2 receptor alpha chain antibody 38 ที่ ใชได ผล ใน steroidresistantacuteGVHDการ ปองกันเปน สิ่ง สําคัญ ที่ สุด ใน TA-GVHD ขั้นตอน ที่ สําคัญไดแก1. การ เลือก หา ผูปวย ที่มีปจจัย เสี่ยง ดัง ตาราง ที่ 1-22. การ ยับยั้ง การ กระตุน หรือ การ แบง ตัว ของdonor2. T-cellsIrradiation 39 จะ สามารถ ยับยั้ง การ แบง ตัว ของdonor T-cells แตตอง เลือก ขนาด ที่เหมาะสม ที่ไมมีผล เสียตอ การ ทํางาน ของ เม็ด เลือด แดง เกร็ด เลือด หรือ เม็ด เลือดขาว ชนิด อื่น ขนาด ที่ แนะนํา ใน สหรัฐ อเมริกา คือ 2,500cGy 40 สวน ใน ญี่ปุน แนะนํา ให ใช ระหวาง 1,500-5,000cGy 41 อยางไร ก็ตาม ไดมีการ ศึกษา พบ วา irradiated redcells ถา เก็บ ไวนานๆ จะ มีผนัง เซลลที่เปลี่ยน ไป มีการ รั่วของ โปตัสเซี ยม และฮโมโกลบิน 42 ดังนั้น จึง ไม แนะนํา ใหเก็บirradiatedredcells นาน เกิน28 วัน ใน เด็ก แรกเกิด หรือ การ ทํา intrauterine transfusion แนะนํา ใหฉาย แสง แลว ใชทันที ไมใหเก็บ ไว สวน ประกอบ ของ เลือดที่ฉาย แสง แลว ตอง มีการ ติด ฉลาก ที่สามารถ เปลี่ยน สีเมื่อถูก ฉาย แสง แลว ให ชัดเจน จาก การ ศึกษา พบ วา ยังสามารถ เกิด TA-GVHD ไดหลัง ฉาย แสง โดย พบ2 รายที่ 2,000 cGy และ1 ราย ที่ 1,500 cGy ซึ่ง อาจ เปน ไป ไดวา เกิด จาก การ ฉาย แสง ที่ไมถูก ตอง 43ขอ บงชี้ใน การ ฉาย แสง แตก ตางกัน ไป ระหวาง ประเทศสําหรับ AABB ของ ประเทศ สหรัฐ อเมริกา ป 1999 44และ BCSH Blood <strong>Transfusion</strong> Task Force ของประเทศ อังกฤษ ป 1996 45 แนะนํา ใหฉาย แสง ใน ผูปวย ทุกราย ที่มีอัตรา เสี่ยง ( ตาม แตธนาคาร เลือด ของ แตละ สถาบันจะ กําหนด)intrauterinetransfusion,directeddonation จาก ญาติหรือ ผูที่มี HLA เขากัน ใน ประเทศ ญี่ปุนจะ มีขอ บงชี้ที่เพิ่ม ขึ้น ไดแก ผูปวย ที่ตอง ผา ตัด หัวใจ และผา ตัด มะเร็ง fresh blood ที่ อายุ นอย กวา 72 ชั่วโมงผูปวย อายุมาก กวา 65 ป ผูปวย ที่มี massive bleedingหรือ severe trauma นอก จาก นี้ยัง แนะนํา ไมใหรับ เลือดจาก ญาติ พี่นอง ผล จาก การ ฉาย แสง สวน ประกอบ ของเลือด ทําให ใน ป 2000-2001 ไม พบ ผูปวย TA-GVHDอีก เลย ใน ประเทศ ญี่ปุนLeucocyte depletion การ กรอง เม็ด เลือด ขาวสามารถ ลด จํานวนT-cells แตยัง มีเซลลที่หลง เหลือ อยูซึ่ง ยัง สามารถ แบง ตัว จํานวน มาก พอ ที่ จะ ทําให เกิด TA-GVHD ได ดังนั้น จึง จําเปน ตอง ใชรวม กับ การ ฉาย แสง 46Pathogen inactivation ไดแก การ ใชแสง ใน การ ฆาเชื้อ โรค ใน เซลล โดย เฉพาะ อยาง ยิ่ง ใน lymphocyte และmonocyte โดย ใช psoralen S59 รวม กับ ultravioletAlight หรือphotoactivephenothiazinedyes พบวา สามารถ ทําใหT-cells หยุด แบง ตัว ไดดวย 47 ทําให ไมเกิด TA-GVHD แถม ยัง ฆา เชื้อ โรค ไดดวย ทั้ง แบคทีเรียและ ไวรัส จึง อาจ ไดประโยชนหลาย อยาง ใน กระบวน การเดียว กันสรุปการ รักษา TA-GVHD ที่ดีที่สุด คือ การ ปองกัน ไมใหเกิด ภาวะ นี้ ใน ปจจุบัน การ ปองกัน ที่เปน ที่ยอม รับ กัน ใน ทุกประเทศ คือ การ ฉาย แสง สวน ประกอบ ของ เลือด กอน นํา มาใหผูปวย อยางไร ก็ตาม มีความ พยายาม ที่จะ หา วิธีใหมๆมา ใชใน สถาน ที่ที่ไมสามารถ ฉาย แสง ได แตผล ยัง ไมคอยดี คง ตอง รอ การ ศึกษา ตอไป บทบาท ของ บุคลากร ทาง การแพทยทั่ว ไป ที่สําคัญ คือ ตอง ตระหนัก ถึง ปจจัย เสี่ยง ในการ เกิด ภาวะ แทรก ซอน นี้ และ ทราบ ขอ บงชี้ที่เหมาะสม ในการ ปองกันThai Journal of Hematology and <strong>Transfusion</strong> Medicine Vol. 12 No. 3 June-September 2002
