Payer, provider, and pharmacy groups have been<br />

involved in testing and evaluating the platform in real<br />

world practice. The current version of the program<br />

was introduced in August 2013, and continues to be<br />

enhanced through user feedback.<br />

Value Delivered<br />

• Over 350,000 patients have experienced the<br />

program in either consumer-focused or HCPdirected<br />

interactions<br />

• A pilot randomized controlled trial of Care4Today ®<br />

Mobile Health Manager among persons living with<br />

HIV demonstrated non-statistical increases in:<br />

Antiretroviral adherence rate, from 57.5%<br />

to 66.8%<br />

––<br />

Proportion of patients achieving high adherence<br />

levels (over 90%), from 17% to 32%<br />

• In an August 2015 study of Care4Today ® Mobile<br />

Health Manager users:<br />

97% of users indicated that the application helps<br />

them take medications as prescribed<br />

––<br />

94% of users indicate that they plan to continue<br />

using the application<br />

––<br />

The overall Net Promoter Score for the application<br />

was 69%<br />

Path Forward<br />

• Based on user and provider feedback from Mobile<br />

Health Manager, the Care4Today team is actively<br />

working on the development of a new platform,<br />

which is set to launch in fall 2016.<br />

• The continuation of value-based care and the shift<br />

toward quality-based measurements will expand the<br />

need for digital health offerings such as Care4Today ® .<br />

While current tools use retrospective generalized<br />

data to improve care, JHI’s long-term goal is to<br />

move toward predictive personalized data. This<br />

will enable a shift toward true coordination of care<br />

and, ultimately, to data-driven care personalization.<br />
