Free Support to Transition from Hospital
to Home - The Home at Last Program
Home at Last is a free service, paid for by the
Waterloo-Wellington Local Health Integration
Network, which provides a warm transition for
older adults 55+, and/or adults with special
needs to settle in at home following a hospital
stay. The purpose of the program is to bridge the
gap between hospital and community, while also
ensuring the proper care for the patient during
the transition. Home at Last works within the
Guelph and Waterloo Wellington area, and is
also able to go beyond to bring patients back
home to the area. The service can be accessed by
the individual either by speaking with a member
of the hospital discharge team or by contacting
Community Care Concepts directly.
Home at Last provides the patient a drive and/
or accompaniment home, as well as support in
settling in at home. This can include grocery or
prescription pick-up, preparation of a small
meal, personal care, or homemaking services.
Afterwards, a follow-up is completed with the
patient by the Care Coordinator to check up on
the patient and to ensure referrals are made to
other community supports if needed. If you
would like more information about the Home at
Last Program, come and check out the CSS
whiteboard display during the month of November
or call the Home at Last Co-ordinator who
can be reached at Community Care Concepts toll
free: 1-855-664-1900.
Food Demonstration
Repeat of CSS Education Session
"Facts and Pitfalls around Estate Plans,
Wills and Executors"
Due to the overwhelming demand for this session,
a repeat is being held on Wednesday, November
22, from 1:00 p.m. - 3:00 p.m. in Room
3. Our guests will host a discussion of things to
consider when drawing up a will, appointing an
executor, designating children or others as coowners
of property or accounts and an explanation
of probate. As well, we'll hear why you
should consider pre-planning your funeral arrangements.
Presented by: Eleanor Ross, Financial Advisor,
Robin Lee-Norris, Lawyer, Debbie Stanley,
Trust and Estate Consultant and Michael Labelle,
Funeral Pre-Planning Director.
Did you know the United Way of Guelph Wellington
Dufferin is the major financial supporter
of GWSA CSS? As one of the 58 agencies the
United Way funds, we simply could not do the
work we do without this support and encourage
you to consider a financial gift to the United
Way as part of your charitable donations. This
year's fundraising goal is $3.5 million for all of
Guelph Wellington Dufferin and the campaign is
in full steam until December.
Come sample seasonal foods and join us for the
final food demonstration this year on Wednesday,
November 22 from 10:00 a.m. - noon in
Room 2. This free event is brought to the
GWSA from the Dietitians with the Guelph
Family Health Team and always is interesting
and informative.
Pre-registration is required. Please call Pat Gage
at 519-837-5696 to sign up.
Diabetes Support Group
