Free Support to Transition from Hospital<br />
to Home - The Home at Last Program<br />
Home at Last is a free service, paid for by the<br />
Waterloo-Wellington Local Health Integration<br />
Network, which provides a warm transition for<br />
older adults 55+, and/or adults with special<br />
needs to settle in at home following a hospital<br />
stay. The purpose of the program is to bridge the<br />
gap between hospital and community, while also<br />
ensuring the proper care for the patient during<br />
the transition. Home at Last works within the<br />
Guelph and Waterloo Wellington area, and is<br />
also able to go beyond to bring patients back<br />
home to the area. The service can be accessed by<br />
the individual either by speaking with a member<br />
of the hospital discharge team or by contacting<br />
Community Care Concepts directly.<br />
Home at Last provides the patient a drive and/<br />
or accompaniment home, as well as support in<br />
settling in at home. This can include grocery or<br />
prescription pick-up, preparation of a small<br />
meal, personal care, or homemaking services.<br />
Afterwards, a follow-up is completed with the<br />
patient by the Care Coordinator to check up on<br />
the patient and to ensure referrals are made to<br />
other community supports if needed. If you<br />

Repeat of CSS Education Session<br />
"Facts and Pitfalls around Estate Plans,<br />
Wills and Executors"<br />
Due to the overwhelming demand for this session,<br />
a repeat is being held on Wednesday, <strong>November</strong><br />
22, from 1:00 p.m. - 3:00 p.m. in Room<br />
3. Our guests will host a discussion of things to<br />
consider when drawing up a will, appointing an<br />
executor, designating children or others as coowners<br />
of property or accounts and an explanation<br />
of probate. As well, we'll hear why you<br />
should consider pre-planning your funeral arrangements.<br />

<strong>November</strong> 22 from 10:00 a.m. - noon in<br />
Room 2. This free event is brought to the<br />
GWSA from the Dietitians with the Guelph<br />
Family Health Team and always is interesting<br />
and informative.<br />

Diabetes Support Group<br />
All are welcome to join our Diabetes Support<br />
Group facilitated by Registered Dietitian, Molly<br />
Roberts. The group meets on the second and<br />
fourth Tuesday mornings of the month (Room 2<br />
from 9 - 10 a.m.)<br />
