GENERAL INFORMATION<br />
Hotel Accommodations: Please make reservations directly with the hotel and NOT <strong>KNA</strong>.<br />
Holiday Inn Hurstbourne Hotel phone number is: (502) 426-2600. Block of rooms held for <strong>KNA</strong><br />
until October 20, <strong>2018</strong>. Room Rate is $119 +tax. We suggest that you register as early as possible to<br />
guarantee a room.<br />
Parking: Complimentary self-parking for the hotel guests is available.<br />
<strong>Conference</strong> Pre-registration: To pre-register, visit kentucky-nurses.org. A pre-registration form of<br />
conference activities and meal functions are also included in this book. PLEASE complete your<br />
online registration or return a printed registration form with your payment to the <strong>KNA</strong> by Monday,<br />
October 22. All participants must check into the registration desk upon arrival at The Holiday Inn<br />
Louisville East, 1325 South Hurstbourne Parkway, Louisville, Kentucky 40222.<br />
Registration: The <strong>KNA</strong> is “going green” this year. <strong>Conference</strong> materials will be available for<br />
participants to download after registration. There will be a $20 fee for those who request printed<br />
materials.<br />

Continuing Education Sessions: To facilitate planning, please complete the registration form<br />
completely (online or printed version). Also, be sure to include either the last 4 digits of your social<br />
security number or your APRN/RN/LPN Licensure Number.<br />
Please include a valid e-mail address. At the end of the program, we will send you an evaluation<br />
email via Survey Monkey. Once you complete the on-line evaluation, the <strong>KNA</strong> office will e-mail a CE<br />
certificate to the email provided. If you do not have an email, you may complete evaluation on-site<br />
and a CE certificate will be mailed to the address provided.<br />
Event Room Changes: Room changes may be necessary. Any changes will be announced at the<br />
business meeting and/or posted.<br />
Breaks: We have several planned breaks. Some of these will be sponsored and recognition will be<br />
given at the break and announced at meetings.<br />
