Free Support to Transition from Hospital to
Home - The Home at Last Program
to be happier and to reminisce, and to hear old tunes
was uplifting!”
Home at Last is a free service, paid for by the Waterloo-Wellington
Local Health Integration Network,
which provides a warm transition for older adults
55+ and/or adults with special needs to settle in at
home following a hospital stay. The purpose of the
program is to bridge the gap between hospital and
community, while also ensuring the proper care for
the patient during the transition. Home at Last works
within the Guelph and Waterloo-Wellington area and
is also able to go beyond this region to bring patients
back home. The service can be accessed by the individual,
either by speaking with a member of the hospital
discharge team, or by contacting Community
Care Concepts directly.
Home at Last provides the patient a drive and/or
accompaniment home, as well as support in settling
in at home. This can include grocery or prescription
pick-up, preparation of a small meal, personal care,
or homemaking services. Afterwards, a follow-up is
completed with the patient by the Care Co-ordinator
to check in on the patient and to ensure referrals are
made to other community supports if needed. If you
would like more information about the Home at Last
Program, come and check out the CSS whiteboard
display during the month of October or call the
Home at Last Co-ordinator who can be reached at
On August 21, Outreach Program participants and
volunteers enjoyed a special summer “Rose Days”
event, made possible with a generous donation from
The Guelph Community Foundation's Community
Grant Program. Named in honour of long-standing
Outreach Program volunteer and co-ordinator,
Wilma Rose, Summer Rose Days programs are arranged
in July and August as an opportunity for participants
to break up the long days of summer and
come to the Evergreen for fun and friendship. Everyone
enjoyed singing along to old summertime favourites
by pianist John Moorehouse, along with a
BBQ lunch prepared by the kitchen. As one participant
put it, coming to the Program was an “invitation
Diabetes Support Group
The Diabetes Support Group runs every second
and fourth Tuesday from 9 to 10 am in Room 2 of
the Evergreen Centre. Our group offers peer support
for the challenges of living with a chronic disease
with input from facilitator and Registered Dietitian
Molly Roberts. We enjoy regular visits from
Diabetes Educators at Diabetes Care Guelph, as
well as other guest speakers, including pharmacists,
social workers and community agencies interested
in diabetes. New members are always welcome
to join our group by coming out to a meeting
or by calling Pat Gage at 519-837-5696.
