Free Support to Transition from Hospital to<br />

Home - The Home at Last Program<br />

Home at Last is a free service, paid for by the Waterloo-Wellington<br />

Local Health Integration Network,<br />

which provides a warm transition for older adults<br />

55+ and/or adults with special needs to settle in at<br />

home following a hospital stay. The purpose of the<br />

program is to bridge the gap between hospital and<br />

community, while also ensuring the proper care for<br />

the patient during the transition. Home at Last works<br />

within the Guelph and Waterloo-Wellington area and<br />

is also able to go beyond this region to bring patients<br />

back home. The service can be accessed by the individual,<br />

either by speaking with a member of the hospital<br />

discharge team, or by contacting Community<br />

Care Concepts directly.<br />

Home at Last provides the patient a drive and/or<br />

accompaniment home, as well as support in settling<br />

in at home. This can include grocery or prescription<br />

pick-up, preparation of a small meal, personal care,<br />

or homemaking services. Afterwards, a follow-up is<br />

completed with the patient by the Care Co-ordinator<br />

to check in on the patient and to ensure referrals are<br />

made to other community supports if needed. If you<br />

On August 21, Outreach Program participants and<br />

volunteers enjoyed a special summer “Rose Days”<br />

event, made possible with a generous donation from<br />

The Guelph Community Foundation's Community<br />

Grant Program. Named in honour of long-standing<br />

Outreach Program volunteer and co-ordinator,<br />

Wilma Rose, Summer Rose Days programs are arranged<br />

in July and August as an opportunity for participants<br />

to break up the long days of summer and<br />

come to the Evergreen for fun and friendship. Everyone<br />

enjoyed singing along to old summertime favourites<br />

by pianist John Moorehouse, along with a<br />

BBQ lunch prepared by the kitchen. As one participant<br />

put it, coming to the Program was an “invitation<br />

Diabetes Support Group<br />

The Diabetes Support Group runs every second<br />

and fourth Tuesday from 9 to 10 am in Room 2 of<br />

the Evergreen Centre. Our group offers peer support<br />

for the challenges of living with a chronic disease<br />

with input from facilitator and Registered Dietitian<br />

Molly Roberts. We enjoy regular visits from<br />

Diabetes Educators at Diabetes Care Guelph, as<br />

well as other guest speakers, including pharmacists,<br />

social workers and community agencies interested<br />

in diabetes. New members are always welcome<br />

to join our group by coming out to a meeting<br />

or by calling Pat Gage at 519-837-5696.<br />
