Accurate work-up of pseudocysts
According to J. Devière (Brussels), pancreatic
pseudocysts are fairly common. One must, however,
assess patient’s findings with a certain degree
of caution, J. Devière warned, “since not
everything that looks like a pseudocyst actually
is one.” In fact, in about 15% of cases, patients
suffer from a cystic tumor (figure 14): Hence, in
no case should drainage be attempted prior to
completion of a full work-up. In addition, pseudo-
cysts very frequently resolve spontaneously. Intervention
is therefore required only in patients
with persisting symptoms. This is of even greater
importance due to the fact that the procedure
itself is not without complications. For example,
bleeding occurs in 2–4% of cases. Retroperitoneal
perforation occurs in about an equal number of
cases. J. Devière also reminded his audience of
the danger of infection.
