parcial a los 3 meses <strong>de</strong> tratamiento, pue<strong>de</strong>n<br />
alcanzar respuesta citogenética completa más<br />
a<strong>de</strong>lante. Por lo tanto, la falta <strong>de</strong> algún tipo <strong>de</strong><br />
respuesta citogenética a los 3 meses <strong>de</strong> tratamiento,<br />
no implica hasta la fecha, modificaciones<br />
<strong>de</strong> las dosis <strong>de</strong> imatinib.<br />
• Estudio molecular: RT-PCR cuantitativa<br />
Constituye el método más sensible para el<br />
seguimiento <strong>de</strong> pacientes con respuesta<br />
citogenética completa. A los 12 meses <strong>de</strong> tratamiento,<br />
los pacientes con respuesta citogenética<br />
completa (RCC), pue<strong>de</strong>n ser subdivididos en 2<br />
grupos: aquellos que lograron una reducción <strong>de</strong><br />
al menos 3 logaritmos <strong>de</strong> los transcriptos bcr-abl<br />
(lo cual se asocia a una sobreviva libre <strong>de</strong> progresión<br />
<strong>de</strong> 98 % a 42 meses), versus aquellos<br />
pacientes con reducción menor a 3 log (sobreviva<br />
libre <strong>de</strong> progresión <strong>de</strong>l 90 % a 42 meses).<br />
La dosis <strong>de</strong> imatinib se incrementará a 600 y<br />
800 mg/día VO en los siguientes casos:<br />
• No obtención <strong>de</strong> respuesta hematológica luego<br />
<strong>de</strong> 3 meses <strong>de</strong> tratamiento<br />
• No obtención <strong>de</strong> respuesta citogenética mayor<br />
luego <strong>de</strong> 12 meses <strong>de</strong> trat.<br />
• Pérdida <strong>de</strong> respuesta<br />
El DASATINIB (131, 132) en dosis <strong>de</strong> 70 mg 2<br />
veces x día VO (ó una dosis <strong>de</strong> 100 mg/día) ó el<br />
NILOTINIB (133), se iniciarán en las siguientes<br />
circunstancias:<br />
• En ausencia o pérdida <strong>de</strong> la respuesta<br />
hematológica y/o citogenética a pesar <strong>de</strong>l<br />
incremento <strong>de</strong> dosis <strong>de</strong> IMATINIB<br />
• En los casos <strong>de</strong> intolerancia a IMATINIB<br />
o Toxicidad no hematológica grado 3 o<br />
mayor, relacionada al imatinib<br />
o Toxicidad hematológica grado 4 relacionada<br />
a imatinib y que dure más <strong>de</strong> 7 días<br />
ROL DEL TRASPLANTE ALOGENICO DE<br />
CELULAS HEMATOPOYETICAS EN FASE<br />
CRONICA<br />
Des<strong>de</strong> el advenimiento <strong>de</strong>l tratamiento con<br />
imatinib, no existe consenso con respecto a la<br />
indicación <strong>de</strong>l trasplante alogénico <strong>de</strong> células<br />
precursoras hematopoyéticas en pacientes con<br />
diagnóstico <strong>de</strong> LMC en fase crónica.<br />
Se sugiere que los pacientes jóvenes, con alto<br />
riesgo según criterios <strong>de</strong> Sokal, y bajo riesgo<br />
asociado al procedimiento <strong>de</strong> trasplante, <strong>de</strong>berían<br />
ser consi<strong>de</strong>rados para trasplante alogénico,<br />
especialmente cuando en el seguimiento se <strong>de</strong>tectan<br />
respuestas citogenéticas y/o moleculares<br />
insatisfactorias.<br />
La indicación es mucho más clara en aquellos<br />
pacientes que progresan hacia fase acelerada<br />
bajo tratamiento con dosis estándar <strong>de</strong> imatinib.<br />
TRATAMIENTO DE LA FASE ACELERADA Y DE<br />
LA CRISIS BLASTICA<br />
Se proce<strong>de</strong>rá a la tipificación <strong>de</strong> la estirpe <strong>de</strong><br />
las células inmaduras, mediante examen<br />
citomorfológico, técnicas citoquímicas y<br />
citometría <strong>de</strong> flujo.<br />
En la fase acelerada y en la crisis blástica se<br />
incrementará la dosis <strong>de</strong> imatinib a 600 a 800<br />
mg/día, o se reemplazará por otro inhibidor <strong>de</strong><br />
tirosina kinasa (dasatinib, nilotinib)<br />

En la crisis blástica instalada se indicará a<strong>de</strong>más:<br />
- Tratamiento <strong>de</strong> sostén.<br />
- Protocolos <strong>de</strong> leucemia aguda (<strong>de</strong> acuerdo a<br />
la tipificación <strong>de</strong> los blastos).<br />
