<strong>Protetik</strong> i <strong>försvagade</strong> bettmed liten konvergensvinkel. När man planerar ocklusionsförändringarav <strong>det</strong>ta slag är <strong>det</strong> viktigt att denya förhållandena provas ut i den temporära konstruktionen.Det är viktigt att involvera patienten iriskbedömningarna eftersom han/hon kanske måsteacceptera viss förändring i både utseende, fonetikoch funktion. Högriskpatienter har emellertid oftaen lång historik med haverier bakom sig och brukarsällan vara svårmotiverade.Den parodontala rörlighetenNär de enskilda stödtänderna i en bro har olikagrad av mobilitet och olika mobilitetsriktning uppstårdragspänningar i cementet. Tändernas rörlighetminskar när de splintas i en bro, men spänningaruppstår i stället i cementet. Stödtändernas rörlighet,antalet stödtänder och fördelningen av dem i denplanerade bron är faktorer som påverkar spänningarnainom cementet. Ju fler negativa faktorer, destokänsligare får lasterna hanteras.Utmattningsfrakturer i cementetFör att få en uppfattning om vilka krafter cementetkommer att utsättas för under funktion rekommenderasatt broar testas i ocklusion, artikulationoch protrusion före cementering (Figur 5). Nären ocementerad bro hålls hårt på plats registrerarfingrarna tydligt vilka krafter cementet kommeratt utsättas för. Tunn ocklusionsfolie mellan tandradernager en visuell markering av <strong>det</strong> man känneri fingrarna och effekten av en justerande inslipningtill gruppfunktion över de tandstödda områdena ochslipning för flackare kusplutning blir uppenbar.Utmattningsfrakturer på implantatbroarFrakturer av proteständer och andra akrylatdelar ärvanligt förekommande inom implantatprotetiken.Det är ingen allvarlig komplikation eftersom bronkan monteras av och skickas på lagning, men <strong>det</strong> ärnog så frustrerande för alla inblandade. För tandläkareoch tekniker ska dessa akutbehandlingar pressasin i redan fyllda tidböcker och patienten måste påkort varsel vara utan sin bro. Även om reparationenär okomplicerad är <strong>det</strong> tidsödande att skruva av bron,temporärt justera patientens gamla protes, transporterabron till och från teknikern samt reparera ochåtermontera den. Patientkostnaden för lagningenblir också tämligen hög.Den platta ocklusionsutformning som beskrivshär reducerar de horisontella krafterna på all protetik.Precis som spänningarna i cementet på tandstöddabroar kan tydliggöras genom att ”provköra”den ocementerade överkäksbron, kan krafterna påskruvförband och implantatbro bedömas genom attman lossar något på brons fästskruvar och låter patientengöra artikulationsrörelser medan bron hållspå plats (Figur 5).Fall 3 (Figur 6 a–e)Denna patient hade implantatretinerade broar i bådakäkarna. Underkäksbron var tillverkad i metallkeramikmed de incisala skären oregelbun<strong>det</strong> utformadeför att skapa ett naturtroget utseende, medan överkäksbronhade ett metallskelett med proteständer. Envertikal överbitning som liknade <strong>det</strong> naturliga <strong>bettet</strong>sbidrog till att överkäksbron snabbt slets ner av <strong>det</strong>skarpa verktyg som underkäksbron utgjorde. I sambandmed ombyggnaden av överkäksbron beslipadesunderkäksbron incisalt så att ”verktygets” vassakanter och hörn avlägsnades och de höga incisivernakortades och ett bra ocklusionsplan skapades. Dettaökar förutsättningen för tandteknikern att skapa enflack kusplutning, stor bettyta och gruppfunktion.Vad säger litteraturen?Det finns förhållandevis lite publicerat om de funktionellakrafternas och ocklusionens betydelse förprognosen för protetiska arbeten. I kliniska uppföljningarav fast protetik nämns sällan något om deintermaxillära relationerna eller om krafter i <strong>bettet</strong>.Förklaringen är uppenbar. Vid kliniska studier är<strong>det</strong> lätt att få fram uppgifter om patienternas ålder,kön, om de har egna tänder eller protes i motståendekäke, hur många extensionsled broarna har et cetera.Det är betydligt svårare, för att inte säga omöjligt,att på ett tillförlitligt sätt mäta krafternas storlek ochriktning på enskilda tänder i <strong>bettet</strong>, ta hänsyn tillstödtändernas distribution, mobilitet med mera.Några artiklar – flertalet av svenska författare – ärvärda att läsas i sin helhet. Henry Beyron, den svenskaprotetikens fader, publicerade tidigt en modell för enterapeutisk ocklusion inriktad på axiell belastning iocklusion som han presenterade tillsammans medfallbeskrivningar med lång uppföljningstid [5–7].Nyman, Lindhe et al har publicerat behandlingsstrategierbaserade på långtidsuppföljningar av fullbroarpå parodontalt mycket <strong>försvagade</strong> bett [8–9].Även de betonade vikten av axiell belastning ochgruppfunktion i artikulation. Med långa uppföljningstiderhar behandlingsprinciper presenterats avbåde Beyron, Nyman & Lindhe som haft stort inflytandepå svensk protetik genom åren. De kliniskariktlinjerna för protetiken i deras artiklar koncentreradespå att reducera de horisontella krafterna.Lundgren, Laurell et al har kopplat samman klinikenoch mekaniken och publicerat både kliniskalångtidsuppföljningar av extensiva broar på riskpatientersamt överskådliga artiklar om biomekaniskaaspekter på fast protetik på tänder och implantat[10–11].De sammanfattar att ”…although failures do occur,they can be kept to a minimum provided thatspecial attention is paid to retention, dimensions andocclusal design of the bridge construction and thatthe periodontal tissues are properly treated”.TANDLÄKARTIDNINGEN ÅRG 97 NR 9 2005 53
