1. _____The Shipper’s name and address must be complete.
2. _____The Shipper number must be the Ground account number or the Express account number.
3. _____The date must be the date of pick-up.
4. _____Shipper must provide a 24-hour emergency response telephone number (including area code) for use in the
event of an emergency involving the HazMat listed.
5. _____The proper shipping name must be the listed exactly as it is in the 49 CFR 172.101 Hazardous Materials
Table – no misspelling or abbreviations.
6. _____The technical name must be listed in parenthesis after the proper shipping name if there is a “G” in column 1
of the HazMat table.
7. _____The hazard class must be listed numerically and the subsidiary hazard class, if applicable, must be listed in
parenthesis after the primary hazard class.
8. _____The identification number must include the UN or NA prefix and a 4-digit number and match the proper
shipping name.
9. _____The packing group must be listed as a Roman numeral I, II, or III. This can be left blank if not applicable.
10. _____The weight must contain the number and the unit of measure, such as lbs. or gal.
11. _____The type DOT labels listed in Column 6 of the HazMat table cannot be abbreviated. Additional information,
such as Limited Quantity, DOT Special Permits, T-#s, Ex-#s, and CA-#s may be listed in this column.
12. _____The receiver’s name, street address, city, state, and zip must be listed here. Ditto marks are acceptable in
this column only.
13. _____List the tracking ID here – 15-digit for the total tracking ID number or the last 8 digits, which is the package
number.
14. _____An authorized representative of the shipper must legibly sign the Shipper’s Certification on the Shipper’s
Hazardous Materials Certification, OP-950. The signature may be mechanically typed or computer
generated.
15. _____This is an incorrect or unauthorized form. Contact your account executive for an updated version.
Please make the necessary corrections to your original OP-950 and return by fax to the number listed
above. The Department of Transportation regulations mandate that you maintain a copy of the shipping
papers for 375 days. Your package may be held or delayed until the correct paperwork is received.
