TEACHER-COACH VERIFICATION (SCHOOL’S REQUIRED)<br />
(INCLUDING ALL SCHOOL COACHES OF COMPETITIVE SPIRIT TEAMS)<br />
FOR A LICENSED TEACHER WHO DOES NOT HAVE A COACHING ENDORSEMENT AND MUST BE<br />
CREDENTIALED THROUGH THE <strong>AAA</strong>-NFHS COACHES EDUCATION PROGRAM<br />
Completing and submitting this form stating that all specified documents are ON FILE AT THE SCHOOL is required<br />
for each licensed teacher who is assigned interscholastic coaching duties and who does NOT have either: (1) an<br />
ADE coaching endorsement, or (2) a physical education teaching license approved by the ADE for coaching (with<br />
written ADE statement of approval or proper licensure code by ADE).<br />
Such teacher-coaches must be assigned professional duties in the district for a minimum of three regular class<br />
periods or equivalent time each day, part of which may be interscholastic coaching duties.<br />

When a licensed teacher has no coaching endorsement or ADE approval to coach the school is required to have the<br />
following confirmed credentials prior to the individual assuming interscholastic coaching duties, including practice.<br />

1. Current teaching license - licensed teacher in the school district where he/she coaches<br />
2. Certificate of Completion – NFHS Fundamentals of Coaching (online at www.nfhslearn.com)<br />
3. Current Certificate of Completion – NFHS First Aid for Coaches (online at www.nfhslearn.com - recertification<br />
required every 3 years)<br />
4. Notification of successful completion of <strong>AAA</strong> state rules exam (non sport specific)<br />
5. Certification – CPR (recertification upon expiration)<br />
6. Certification – AED (defibrillator - recertification upon expiration)<br />
7. Concussion Training – NFHS Concussion Course (online at www.nfhslearn.com)<br />
8. Heat Illness Training - available through <strong>Arkansas</strong>IDEAS at http://ideas.aetn.org/<br />
9. MSRA Training – Requirement begins for the <strong>2012</strong>-2013 school year<br />
10. Certification of Completion - NFHS Sport Specific Fundamentals course, if available, for each sport coached –<br />

Current AACCA Safety Certification (4-yr certification – recertify by taking the course again<br />
NOTE: A coach of more than one sport must take the sport-specific fundamentals course for each sport coached. For a multi-sport coach, if<br />
one sport’s fundamentals course is available and the other is not, the coach must also complete the available sport-specific course(s) AND the<br />
Teaching Sport Skills course.<br />
