test <strong>for</strong> publication bias), no statistical analysis and<br />

poor reporting of the intervention. No reporting<br />

of adherence to <strong>screening</strong> protocols limits the<br />

interpretation of identification rates. These study<br />

limitations weaken overall findings and ultimately<br />

reduce the strength of evidence of these primary<br />

studies. The main limitations arising from the<br />

primary studies are failure to measure or lack of<br />

power to detect actual morbidity (in the case of<br />

the study by Thompson and colleagues 169 ) and the<br />

generally weak study designs.<br />

Discussion<br />

Despite the finding that interventions in primary<br />

care settings produced overall a trend <strong>for</strong><br />

increased identification and other activities aimed<br />

at reducing morbidity and mortality, there is<br />

insufficient evidence of effectiveness. The most<br />

methodologically robust study showed least effect<br />

on identification rates.
