• Multiple gestation without complications<br />

High-Risk Postpartum Patients<br />

• Premature or multiple-birth delivery<br />

• Complications of labor, delivery, or puerperium<br />

• Breastfeeding problems or complications<br />

• Delivery resulting in infant death or severe congenital anomalies with<br />

need for emotional and psychological support<br />

• Current or history of depression or mental health disorder<br />

• Current or history of medical or obstetrical complication<br />

• Needing additional teaching (parenting, child care, etc.)<br />

• Under 19 or over 35 years of age<br />

• Single parent or patient without family support<br />

• Current or history of alcohol or substance abuse<br />

• Current or history of previous domestic violence<br />

• Anemic (< 9 Hgb)<br />

High-Risk Neonate, Infant, or Child<br />

• Neonates with complications of labor, delivery, or congenital anomalies<br />

• Premature infants or low birth weight (equal to or less than 2,500 grams<br />

at birth)<br />

• Infants of multiple-birth delivery<br />

• Failure to thrive or lack of other normal physiological and mental development<br />

• Evidence of poor maternal-infant bonding<br />

• Infants born to mothers with a history of alcohol or substance abuse<br />

• Patients in families with history of domestic violence<br />

• Infant child of teen parent(s) in need of PHN intervention<br />

• Patients whose parents have cognitive or learning disabilities<br />

Noncompliance With Medical Treatment<br />

• Potentially life-threatening or adverse consequences<br />
