Three<br />

The Chief Medical Examiner recommends that Southeast Child and Family<br />

Services maintain its case records in accordance with provincial standards,<br />

including placing on its files ongoing summaries <strong>of</strong> events in the lives <strong>of</strong> the<br />

families and children they serve.<br />

The Chief Medical Examiner recommends that Southeast Child and Family<br />

Services review its policies on the placement and replacement <strong>of</strong> children in<br />

the foster care system, with a view to how multiple placements can be<br />

minimized.<br />

The Chief Medical Examiner recommends<br />

that Southeast Child and Family<br />

Services, with the assistance <strong>of</strong> the Southern Authority, review with its<br />

foster care department as well as its foster parents the placement and<br />

replacement <strong>of</strong> foster children in their homes.<br />

The Chief Medical Examiner recommends that the Child Protection Branch<br />

and the Authorities ensure the province's foundation standards include a<br />

provision for pre-placement visits when children are placed in nonemergency<br />

or replacement<br />

Family Services, in the interim, ensure that children in care are provided<br />

with pre-placement visits (in non-emergency or replacement situations) prior<br />
