discussion about what happened and what changes were required to reduce the
likelihood of such tragedies occurring in the future. It was no surprise to me or my
colleagues that such an event would attract public scrutiny or that the public servants
involved would be expected to come forward to explain what happened and discuss
strategies for avoiding such outcomes in the future.
16. In the end the agency, its staff and the community learned from the experience
and I did not perceive any negative impact of the process on the ability of our staff to
continue to provide services to the communities we served. The public inquest gave
participating staff members an opportunity to present to the community and decisionmakers,
their perspectives about the specific case, the child welfare system and the
challenges faced by the men and women who administer it.
