Introduction: Benzyl alcohol (BA) is a common preservative in foods, cosmetics and medicines. All heparin solutions funded in New Zealand for anticoagulation during haemodialysis contain 1%w/v BA while in other countries, preservative free heparin is available. WHO recommends a maximum exposure of BA/benzoic acid (BBA) of 5 mg/kg/day 1 . In haemodialysis patients, BA exposure from heparin is estimated to be in the order of 56-380 mg/week, but it is unknown whether this contributes to systemic exposure. The aim of this study was to investigate the contribution of BA in heparin to plasma concentrations in patients undergoing long-term haemodialysis. Methods: A high pressure liquid chromatographic (HPLC) method was developed for analysis of BA and its metabolites (benzaldehyde, benzoic acid and hippuric acid) in plasma and dialysate fluid. The Shimadzu HPLC comprised a LC-10ATvp solvent pump, SIL10ADvp autosampler, SPDM- 10Avp diode array detector and SCL- 10Avp system controller (Shimadzu, New Zealand). The column was a Luna C18, 5μm, 250x3mm i.d. (Phenomenex, New Zealand), maintained at 30°C. The mobile phase was acetonitrile:0.1%v/v acetic acid in water (25:75% v/v) at a flow rate of 1 mL/min. Detection was at 215nm and injection volume was 20μL. Plasma samples (250μL) were spiked with 50μL internal standard solution (100ug/mL benzocaine), then perchloric acid (30% v/v) was added to precipitate protein. Samples were centrifuged at 10 000 rpm, and the supernatant was transferred to HPLC autosampler vials for analysis. Dialysate fluid samples were analysed directly. Haemodialysis patients (n=10) were enrolled in the study. Heparin 1000 IU/mL (containing 1% w/v BA) was administered during haemodialysis. Blood was collected prior to entry into the haemodialysis machine before the session, at 1 hour and at the end of the session (4 or 5 hours). Dialysate samples were collected at 0, 10, 30 minutes and 1 and 2 hours. Plasma was immediately separated from blood and both plasma and dialysate samples were stored at -20 o C prior to analysis. Ethical approval was from the Lower South Regional Ethics Committee (New Zealand). Results: BA was separated from its major metabolites with a retention time of 4.6min. Recovery of benzoic acid and hippuric acid from plasma was greater than 80%, but was 65 to 75% for benzoic acid and 51 to 56% for benzaldehyde. Peak area ratios (Analyte/Internal standard) were linear over the concentration range 1 to 100 μg/mL for BA and metabolites (R²>0.99).
