A. Arbitrary Adjustment of Occlusion Rims<br />

Prior to determining the occlusal vertical dimension of occlusion, the occlusion rims should<br />

be adjusted separately to using average dimension and guidelines. This arbitrary adjustment<br />

provides a ‘starting point’ for adjustments and should be fairly rapid. It should normally not<br />

take longer than 15 minutes per arch.<br />

1. Maxillary Occlusion Rim Adjustment<br />

- The maxillary wax rim should be slightly facial to the ridge to compensate esthetically<br />

for facial resorption of the bone<br />

- Anterior height should be 1-2 mm below the lip when the patient slightly smiles<br />

- The rim should touch the wet line of the lower lip, when the "F" or “V” sounds are<br />

spoken (have the patient count from 50 to 60)<br />

- The nasolabial angle should be approximately 90°, and the lips should be unstrained,<br />

with the vermilion border showing<br />

- The anterior-posterior orientation of the occlusal plane should be parallel to the alatragus<br />

line (Fox plane)<br />

- The mediolateral orientation of the occlusal plane should be parallel to the pupils<br />

2. Mandibular Occlusion Rim Adjustment:<br />

- The mandibular wax rim should be centered over the middle of the ridge to maximize<br />

stability, which is usually compromised in the mandible<br />

- The posterior height of the wax rim should intersect with a point l/2 - 2/3 up the<br />

retromolar pad<br />

- The anterior height of the wax rim should be even with the corners of the mouth when<br />

the lip is relaxed<br />

- The maxillary wax rim should have 1-2 mm horizontal overjet anteriorly and posteriorly<br />

when the patient is in a centric position<br />

-The patient should demonstrate unstrained lips, with the vermilion border showing<br />

B. Establishing the Occlusal Vertical Dimension<br />

Physiologic Rest Position (PRP) - at rest, when the lips are barely touching, the<br />

occlusion rims should not touch<br />

Interocclusal Distance (ID) is the space between opposing occlusal surfaces or wax rims<br />

while a person is in the physiologic rest position - usually 2-4 mm.
