Preventive Nursing Division - The preventive programs of the Public Health Nursing Service provide services in the areas<br />
of maternal/child health, chronic health issues, and communicable disease control. The goal of preventive services is to<br />
promote the optimum level of health for all residents of <strong>Oswego</strong> <strong>County</strong> through comprehensive preventive services:<br />
Home visits by Registered Professional Nurses (preventive and skilled care)<br />
Communicable Disease Control (surveillance of all communicable disease)<br />
Tuberculosis Control Program<br />
Sexually Transmitted Disease Screening<br />
HIV testing<br />
Immunization Clinics – state required immunizations for all ages, plus flu and pneumonia shots<br />
Lead Poisoning Program (prevention, investigation, and abatement)<br />
Health Education Programs (programs available to the community on a variety of health issues)<br />

Physically Handicapped Children’s Program is designed to help children (0-21 years) who have or are<br />
suspected of having a serious or chronic physical condition and who also require health and related<br />
services of a type or amount beyond that required by children generally.<br />
The PHC Program can help families:<br />
Access insurance if they do not already have coverage<br />
Provide guidance to those families, especially if a child is being seen by many health care providers or would like<br />
help with referrals to other community programs that may benefit the child and family<br />
Set up any needed diagnostic testing and evaluations for their child suspected of having a physically<br />
handicapping condition<br />
Offset any medical bills that are not covered by the families regular health insurance<br />
Help families meet any deductibles they may have<br />
Pay for hearing aids and other durable medical equipment<br />
A staff member from PHC will help parents through the application process to determine if they qualify financially for<br />

Early Intervention <strong>Services</strong><br />
Provides service coordination services to infant and toddlers (birth to 3) with developmental delay or disabilities and their<br />
families. The service coordinator can help the family obtain developmental evaluations and direct services to help ensure<br />
that children with delays or disabilities reach their full potential. The service coordinator will also help the entire family<br />
receive services according to their needs. The service coordinator can help the family access other services such as parent<br />
support groups, funding assistance, such as PHCP, Medicaid, SSI, WIC and advocacy services. This program is a very<br />
family centered program.<br />
The Health Department is serving as a single point of entry for this program. To make a referral to this program or for<br />

Child Find The main focus of the Child Find Component of the Early Intervention Program is to ensure children are<br />
engaged in primary health care where they will receive periodic developmental surveillance and screening. The Child Find<br />
staff work in partnership with area physician’s to ensure that children who are at risk for developmental delays or<br />
disabilities, receive appropriate developmental monitoring. The Child Find program can help parent’s keep track of their<br />
child’s development through periodic contact with program staff, through parent questionnaires and by maintaining contact<br />
with their primary physicians.<br />

3-5 Preschool Special Education - Provides educational services to children (aged 3-5) with developmental delay or<br />
disabilities as recommended by their local school district Committee on Preschool Education (CPSE). As <strong>Oswego</strong> <strong>County</strong><br />
is the payor of this program, this office provides coordination with the county's nine school districts, providers of services<br />
(both through <strong>Oswego</strong> <strong>County</strong> BOCES and independent contractors), transporters of services, Head Start, and the New<br />
York State Education Department.<br />
