Editor’s letter 3 – February/March 2011 Prostate cancer management: a continuing challenge Accounting for between 6 - 10 % of cancer deaths in men, prostate cancer (PCa) is the second most frequently diagnosed cancer in the West, with around one in six men in the developed world eventually being diagnosed with the disease. While older age and family history increase the risk for PCa, there are no established modifiable risk factors; reduction in the mortality rate is dependent on early diagnosis and timely treatment. Although a routine blood test revealing an elevated prostate-specific antigen (PSA) level may detect PCa before symptoms have developed, despite its name this marker is actually far from specific. It may be elevated in several benign conditions, with the result being unnecessary biopsies. In addition, a considerable proportion of men with low levels of PSA below the cut-off value of 4 ng/mL actually have prostate cancer. Various research groups have thus been looking for more effective methods of measuring PSA as well as for more specific and sensitive markers for diagnosis and prognosis of the disease. Over the past decade several groups from prestigious institutes have reported that the use of prostatespecific antigen velocity (PSAV), which involves the change in PSA level over a specified time interval, is a more effective strategy for risk prediction, diagnosis and prognosis of PCa. Even in cases with total PSA levels below the cut-off value, and certainly with values between 4 and 10 ng/mL, an elevated PSAV above 0.4 ng/mL/year, has been reported to be a strong predictor of prostate cancer risk. An elevated PSAV was found to be significantly more likely to result in aggressive disease and ultimately death from PCa. However a recent comprehensive US study, reported in the Journal of the National Cancer Institute and involving 5,000 men, found that even a rapid increase in PSA level was not necessarily linked to prostate cancer if the actual absolute level of the antigen was low. The researchers concluded that the PSA level varies naturally over time, so that men whose PSA level suddenly increases should have a repeat test rather than undergo a possibly unnecessary biopsy. However the controversy surrounding PSAV and its value in PCa screening can probably be explained by the fact that the many different PSA assays available are not interchangeable, nor are they necessarily referenced to the same laboratory standards. To give reliable results, longitudinal monitoring of PSA must use the same assay and standard each time a subject is tested. Such individual risk assessment based on the doubling time www.partec.com of relevant markers, as well as the development of a panel of biomarkers incorporating PSA isoforms, will hopefully not only reduce unnecessary biopsies, but will have a significant impact on the overall prostate cancer mortality rate.
