APPENDIX I - Equipment Investigation Report<br />

Section 4-101 of the Colorado Retail Food Establishment Rules <strong>and</strong> Regulation specify all equipment,<br />

utensils <strong>and</strong> single-service articles shall be fabricated with safe materials; be of commercial design, that is<br />

certified or classified by an American National St<strong>and</strong>ards Institute (ANSI) accredited certification<br />

program, such as the National Sanitation Foundation (NSF), Underwriters Laboratories (UL) sanitation<br />

st<strong>and</strong>ards, Environmental Testing Laboratories, Inc. (ETL) sanitation st<strong>and</strong>ards, Baking Industry<br />

Sanitation St<strong>and</strong>ards Committee (BISSC), or other comparable design criteria as approved by the<br />

Department during a st<strong>and</strong>ardized equipment review.<br />

If a <strong>retail</strong> <strong>food</strong> <strong>establishment</strong> intends to have any equipment, utensils <strong>and</strong> single-service articles approved<br />

by the Department, the approval will be based upon submission of the following information to be<br />

provided to the local public health agency <strong>and</strong>/or the Colorado Department of Public Health <strong>and</strong><br />

Environment for evaluation.<br />
